本研究では、日本とオーストラリアにおける費用効果分析の閾値を仮想評価法(CVM)の手法を用いて測定した。費用効果分析において、いくら以下なら費用対効果に優れるのかその閾値を巡っては様々な議論がある。イギリスでは£20,000-£30,000perQALY、アメリカでは50,000US$-100,000US$perQALYと言われているが、明確な根拠はない。日本では、未だあまり議論されていないのが現状である。そこで本研究ではインターネットパネルから無作為抽出した1000人の回答者に対し、追加的に1QALY獲得する際の支払い意志額(WTP)をたずねた。回答者は性別と10代刻みの年齢で層別し、すべての層の回答者数が等しくなるよう調整した。WTPは2段階2項選択法を用いてノンパラメトリックに推定した。本研究の結果から1QALYあたりのWTPは500万円(日本)、64,000AU$(オーストラリア)であることが明らかになり、また効果の割引率は年率6.8%(日本)、1.9%(オーストラリア)であった。これらの値は欧米で参照される値と近い値であり、慣習的に用いられていた閾値はWTPの観点からも正当化されると考えられた。また割引率は一般に3%ないしは3.5%が用いられているが、今回得られた値もそれらに近いものであった。これらの結果から日本では500万円前後が閾値の目安になると考えられた。別途推定した個人の支払い意志額と人口統計学的背景との関連を分散分析モデルで解析したところ、世帯の年収や教育歴が高いほど支払意志額が高くなる傾向があることが、各国に共通の傾向として明らかになった。
