医療経済評価における効果としての健康の価値を評価する方法として、QALY(Quality Adjusted Life Years)を算出するための効用値の測定やWTP(Willingness to Pay)を用いた便益の測定方法について、特に医療における公平性や利他性を考慮した方法の検討を行った。関連する文献レビューの結果から、利他性を測定する方法として、WTPを用いた方法が多く用いられており、個人が全額自己負担で支払う場合のWTPと、税金などにより賄うための利他性が考慮されたWTPが重要と考えられた。またPTO(Person Trade Off)などのQOL測定法を用いて個人の健康状態の変化と集団の健康状態の変化の価値付けを行い、検討しているものもあった。そこで、健康状態の評価に関する調査としてA大学病院の看護師を対象にWTPを用いて稀少疾患である慢性骨髄性白血病の遺伝子診断および治療に関する便益の推定を行った。結果として薬剤の適性を判断するための遺伝子診断および薬剤の双方について高い値であった。また、EQ-5Dの5次元、3段階の健康状態記述方法を用いて利他的効用を測定する面接調査を実施した。対象は看護を専攻する学生とし、一部は実務経験があるものであった。4つの仮想的な健康状態が改善する医療を想定し、それぞれについて自分が医療を受ける場合と自分以外の人がその医療が必要な場合の治療法の優性順位やWTP,PTOの質問をした。想定した医療はいずれもQOL評価値でおよそ0.2改善するものであったが、自分が医療を受ける場合のWTPの中央値は10万円から30万円程度であり、自分以外の人に対する医療についてのWTPの中央値は1000円から2000円と自分が医療を受ける場合の1%程度であった。
