本研究では、(1)慢性疾患疾病管理の視点に立ち、将来的には「看護師が一定の区域を受け持ち、アウトカム・マネジメントの手法を用いてその地域住民の健康管理に責任を持つ仕組み」を構築すること、及び(2)家族を含めた総合的な健康管理を行う機能をもつ看護師(Family Health Nurse:以下、FHN)を育成し、FHNに(1)を実施する機能を持たせることを目標に、その提言のための基礎研究(基盤作り)を行った。広島市基町団地内の高齢者立ち寄り所「ほのぼの基町」の中に「健康管理室」を設置し、看護師を配置し、週3日、活動を行った。慢性疾患管理の仕組みづくりについては、(1)4月〜5月にかけて住民を個別訪問し、疾病管理に関するニーズ調査を行った。また、(2)基町地区「高齢者見守り体制づくり事業運営委員会」と協働し、見守り個別訪問と閉じこもり者を拠点「健康管理室」に連れ出し、基町地域包括支援センターと連携してケアマネジメントと連動する形とした。さらに、(3)地域の開業医との連携体制を構築し、受診後、本拠点により保健指導を行い、それを開業医にフィードバックする仕組みも構築した。「健康管理室」を訪れた住民は、約400人で、COPD・慢性腎不全・狭心症・不整脈・高血圧症・肝炎・糖尿病・精神疾患・気分野落ち込みなど多岐にわたる疾患や状態に対して、ヘルスアセスメントの実施、定期的保健指導(病気の理解、服薬管理、療養管理、日常生活管理、栄養・運動指導、ストレスマネジメントなど)、また、家族間の調整やケースマネジメントを実施した。新しい仕組みであることから、地域住民の理解を得るための困難を克服しつつ、徐々にではあったが、拠点に対する住民のニーズは高く、今後、地域における新しい看護ケア提供の仕組みとなると考える。ただし、どのように運営経費を生み出す仕組みを構築するかは、政策提言も含めて今後の課題である。
