本研究は、学校看護師制度や学校健診のないバングラデシュで試験的に学校看護師を養成、小学校に配置し、健康診断と科学的根拠に基づく健康教育を実施し、児童の健康への意識を高め、児童が親や地域のアドボケーターになることで、地域全体の感染症や栄養不良を発症する者の減少を狙うものである。学校保健の仕組みについても検討を行う。令和４年度は、令和３年度に開始した地域での試験を継続し、終了した。フィールドとして選定した農村部（Ekhlaspur地区とJahirabad地区）の4つの小学校の全学童（n=604）を対象に、健康診断と質問紙調査を実施した。健康診断の内容は、身体計測（栄養状態の計測を含む。）、血液検査、視力検査、尿検査（尿糖、尿蛋白）、便検査（寄生虫検査）である。健康に対する意識・知識、日常生活の健康行動、生活の質（QoL）についても質問紙調査を行った。(1)疫学調査結果：一定割合の低栄養の児童の存在が確認できた。また、健康に関する知識の程度や清潔に関する行動においても手を洗わない児童の割合などが明らかとなった。小学校全学年を対象とした幅広い健診の実施はバングラデシュでは初めてで価値あるものであり、結果については現在論文投稿中である。(2)介入研究結果：同じ対象者に、クラスター非無作為化比較試験を実施した。研究期間は、2021年9月から2022年9月までの13ヶ月間であった。子どもたちは、介入群（n = 314）、対照群（n = 290）にランダムに割り振られた。結果、学校看護師が主導する健康教育プログラムは、小学生のBMIや栄養失調に関する意識と知識レベルを大幅に改善し、食行動も改善させた。本研究により、小学校における学校看護師の存在が子どもの栄養状態を改善し、将来的に子どもの罹患率や死亡率を減らすことに寄与する可能性があることが明らかになった。現在、論文投稿中である。
