目的:脳卒中患者が救急搬送される広島市内の急性期2病院に脳卒中の専門的教育を受けた看護師(脳卒中専門看護師)を配置し、配置前後の臨床指標(Clinical Indicators : CI)を測定・比較した。また、看護師への教育効果も測定した。方法:CIは、到着時・到着から24時間・48時間・72時間・1ヵ月後・退院時点のmodified Rankin Scale (mRS)・Japan Stroke Scale (JSS)・Japan Coma Scale (JCS)の値、血圧(収縮期・拡張期)、脈拍、到着から検査・治療開始・入院までの時間である。専門的教育は、2施設の看護師合計6人に6ヶ月間16回(21時間)の講義と国内の先駆的病院での実習を2日間実施した。研究期間及び対象:対照群(介入前)平成15年10月〜平成16年9月までに2病院に入院し、退院した脳卒中患者合計120人。介入群(介入後)平成16年10月〜平成17年1月の間に2病院に入院した脳卒中患者合計33人。倫理:病院及び患者・家族から調査協力の同意書を得た。結果:患者のベースライン比較は、基本属性・対照群(男性55.8%,年齢72.0±12.65,脳梗塞63.3%,脳出血28.3%)と介入群(男性54.5%,年齢72.6±8.53,脳梗塞63.6%,脳出血33.3%)で有意差はなかった。到着時、JCS2.68±2.55(対照群)、2.91±2.54(介入群)、JSS24.46±5.80(対照群)、9.49±4.95(介入群)、mRS3.80±1.35(対照群),4.24±1.17(介入群)であった。脳卒中専門看護師の配置によってJSSの実施率は9.2%から66.7%まで、JCSは9.2%から100%まで上昇した。脳神経領域の知識テストの正解率も48%から62%、43%から73%まで上昇した。到着から検査や治療までの時間も10分程度短縮され、mRSとJSSの一致度(相関)も高く、看護師の症状の観察力の向上が示された。脳卒中専門看護師配置効果としてCIの変化は、観察能力の向上と到着から検査(CT, ECG)・薬剤開始までの時間の短縮による治療の迅速化と生命リスクの低下に現れた。しかし、機能レベル(mRS, JSS, JCS)及び血圧・脈拍については部分的にしか有意な改善は観察されなかった。脳卒中急性期の臨床アウトカムの向上を意図したこの介入研究のデザインの有効性が示唆された。

Objective: To measure and compare the clinical indicators (CI) before and after the deployment of stroke nurses in the acute phase of stroke in 2 hospitals in Guangzhou. The results of nursing education are measured. Methods:CI, arrival time, arrival time, 24 time, 48 time, 72 time, 1 month later, modified Rankin Scale (mRS), Japan Stroke Scale (JSS), Japan Coma Scale (JCS), blood pressure (contraction period, expansion period), pulse beat, arrival time check, treatment start, admission time. The total number of nurses in the department is 6, and the lectures are 16 times (21 hours) in 6 months. The training is carried out in 2 days in advanced hospitals in China. Study Period and Subjects: A total of 120 stroke patients were admitted to and discharged from 2 hospitals from October 2015 to September 2016. Intervention group (after intervention) 33 stroke patients were admitted to 2 hospitals from October 2016 to January 2017. Ethics: The consent form for cooperation in the investigation was obtained from the hospital and the patient's family. Results: The comparison of the patients 'basic attributes was significant (male 55.8%, age 72.0±12.65, infarction 63.3%, hemorrhage 28.3%) and the intervention group (male 54.5%, age 72.6±8.53, infarction 63.6%, hemorrhage 33.3%). By the time of arrival, JCS 2.68 ±2.55(control group), 2.91±2.54(intervention group), JSS 24.46 ±5.80(control group), 9.49±4.95(intervention group), mRS 3.80 ±1.35(control group), 4.24 ±1.17(intervention group). JSS implementation rate increased from 9.2% to 66.7%, and JCS increased from 9.2% to 100%. The correct rate of knowledge in the field of psychology increased from 48% to 62% and from 43% to 73%. The time taken to detect and treat the disease was shortened by 10 minutes, the consistency (correlation) of mRS and JSS was increased, and the observation ability of the caregiver was increased. Stroke care provider configuration results in changes in CI, improvements in observation capabilities, reductions in time to start therapy, and decreases in life expectancy. The function of the system (mRS, JSS, JCS) and the pulse rate of the blood pressure are improved intentionally. The clinical significance of stroke in acute phase is discussed.
