项目摘要

実験胃潰瘍の底部結合組織の収縮活性について研究を行い、以下の実績を得た。1.実験胃潰瘍である酢酸潰瘍を作製し、その潰瘍底部から肉芽組織を含む全ての結合組織の単離に成功した。2.その単離した潰瘍底部結合組織が、セロトニン、ブラジキニン、プロスタグランジンFla、カルバコール等の薬物によって収縮することを証明した。またこの潰瘍底部結合組織が平滑筋層を全く含んでいなかったことにより、確認された収縮反応は肉芽組織そのもの、主に筋繊維芽細胞もしくは繊維芽細胞を介して認められるものであることが推察された。3.我々は、既にラットに作製した酢酸胃潰瘍が非ステロイド系抗炎症薬であるインドメタシンの連続投与によってその治癒が遅延されることを報告している。このインドメタシンを投与した動物の潰瘍底部より結合組織を単離しその収縮活性を検討したところ、対照群と比較してその活性が有意に減弱していることが判明した。4.組織学的な検討よりもインドメタシンが収縮反応を抑制していることが明らかとなった。また、インドメタシンを投与した群の潰瘍底部の肉芽組織は、異常かつ過剰なコラーゲン繊維の走行が観察され、肉芽組織の繊維化が高度に進行しているものと考えられた。5.インドメタシンの投与によって認められた肉芽組織の繊維化をステロイド系抗炎症薬の一つであるプレドニゾロンを処置することによって分解すると、胃潰瘍の有意な縮小が認められた。このことは、インドメタシンによる胃潰瘍の治癒遅延の機序に高度な繊維化を介した肉芽組織の収縮抑制が関与していることを示すものである。以上より、胃潰瘍の治癒は肉芽組織の収縮によって進行するものであることが明白となり、この点に関するより詳細な検討は、胃潰瘍の治癒を論じる上に於いて重要な知見を提供しうるものであると考えられた。
