1.研究目的:福祉施策としての軽費老人ホームのB型およびケアハウスと、住宅施策としての各種高齢者住宅の間の区別と関連付けは、今後の老人施策の体系化の上で重要な課題の一つである。本研究は、これら2種類の施策の効果と効率を、主として費用・効果分析の手法を用いて比較することを通して、今後の老人福祉施設および高齢者住宅のあり方に有益な示唆を得ることである。2.研究方法:(1)軽費老人ホーム利用者(「軽費」)と高齢者住宅入居者(「住宅」)に対して、機能的健康度、うつ傾向、モラール、余暇活動、社会的支援、社会的交流、居住性・環境満足度等に関する調査を行い、その結果を両対象者の間で比較した。科研費申請時の計画では対象者全員に対する面接調査を予定したが、実際の補助金額ではそれが困難となり、一部の対象者に対してのみ面接、他は郵送調査によった。このため回収率が低く有効回答は、「軽費」がB型53人およびケアハウス45人の計98人、「住宅」が民間住宅借り上げ型38人、シルバーピア(東京都単独事業の高齢者住宅)51人の合計89人から得られたにとどまった。(2)費用に関しては、上記の対象者が居住するホーム及び住宅8カ所の管理者に、一人当たり運営費の算出を依頼した。研究結果:利用者・入居者調査についての1次的分析の結果は次の通りである。(1)老研式活動能力指標により測定した機能的健康度は、「軽費」より「住宅」の方が有意に良好であった。(2)余暇活動とCES-D尺度によって測定したうつ傾向は、両者の間に有意な差はなかった。(3)モラール、社会的支援、社会的交流の程度のいずれについても、「軽費」より「住宅」の方が良好であった。(4)居住性・環境満足度は、「軽費」と「住宅」の間の差よりも、開設年による差の方が大きかった。なお費用分析は、データ収集が遅れたので今後行う。
