本研究の目的と用いた研究方法は次の通りである。1在宅障害老人とその家族がもつ客観的な保健・福祉ニ-ズの測定。われわれの一連の先行研究を通して開発してきた福祉ニ-ズの測定方法をさらに改善し,主として痴呆の症状や問題行動に関連した保健ニ-ズをも含めたより総合的なニ-ズの測定方法を作成した。2上の客観的なニ-ズ状況のもとで介護者が抱く負担感,サ-ビス希望,およびその状況に対処するために介護者がとる態度および行動の測定とその要因分析。これらの測定のための尺度も,われわれの先行研究で作成を試みてきたが,ここ数年,老年学の分野ではこれらの測定方法および統計的解析方法が急速に進んでいるので,それらを参考にしてかなり大幅な改善を行なった。調査対象は,平成5年度からすべての市町村で老人保健・福祉計画が立案されることになったことに鑑みて、これまでほとんど対象とされてこなかった町村部の在宅障害老人とした。調査は、われわれの先行研究と同様,2段階で行われた。すなわち,T県の2町1村を対象地域にして、まず無作為抽出した4,650人の在宅老人を対象に郵送法によるスクリ-ニング調査を行い,321人の障害老人が選ばれた。次にこれらの老人の主介護者を対象に訪問面接法による本調査を行った結果,最終的に272人の有効回答票が得られた。障害老人の発現率は,8.5%であった。この率は,われわれが東京で行なった2つの先行研究の結果と殆んど変らない。しかし、先行研究では主として身体的障害に着目しているのに対して,今回は相当詳しい精神的障害のチェックリストを用いて精神的障害のみの老人も選び出されているので,身体的障害老人の発現率は大都市より低いと言える。なお本格的なデ-タ解析は、研究スタッフの所属研究機関における研究に引き継がれる。
