重度視覚障害者(児)のQOLの向上を目指した医療で、眼科医および視能訓練士など眼科医療関係者の任務を明らかにする研究において、平成5年度の助成金によって以下の成果を得た。1.重度視覚障害者(児)の実態調査:1993年4月に開設した川崎医科大学附属病院眼科外来での眼科リハ・クリニック(ロービジョンクリニック)に登録された障害者(1993年9月現在)は122名;女66名、男56名であった。年齢は平均47.3歳±偏差26.1歳で、40歳以上が83名(68.3%)で60歳代が最も多かった。原因は網脈絡膜の先天素因または糖尿病など全身疾患によるものが44名(36.1%)で、次いで強度近視など先天素因による眼球全体の疾患が12名(9.8%)であった。さらに、実際の眼科外来での重度障害者の頻度を調べると、ある1週間の眼科外来の受診総患者856名(女501名、男355名)中、25名(女15名、男10名);2.9%と言う多数の障害者が候補として占め、その多くが眼科リハ・クリニックを望んでいることが明らかとなった。2.眼科リハ・クリニックの実際:障害者の日常生活や勤務状況、家庭環境や本人自身および周囲の希望や問題点を聴取し、クリニックでの指導を希望する例に眼科医と視能訓練士および眼科生活訓練指導が出来るソーシャルワーカーと協議しながら、適宜、単眼鏡や卓上拡大レンズ、拡大読書器、遮光レンズなどの処方および使用訓練、さらには簡単な歩行指導、白杖の使い方、生活指導、社会保障制度や障害施設の紹介などを行った。さらに拡大読書器による読書中の眼球運動を記録して晴眼者と障害者の違いを明らかにし、第47回日本臨床眼科学会(1993年10月)および第4回日本視能矯正学会(同年11月)で、それぞれ発表および教育講演を行った(いずれも原著投稿中である)。

The following results were obtained from the research on the upward direction of QOL for severely visually impaired persons (children), medical care, ophthalmology doctors and vision trainers, and responsibility for ophthalmology medical relations. 1. Survey of Severe Visual Impairment: Kawasaki Medical University Affiliated Hospital opened in April 1993. There were 122 registered visual impairment patients (66 females and 56 males). The average age is 47.3 years ± deviation is 26.1 years old, 83 years old (68.3%) and 60 years old. 44 cases (36.1%) of diabetes mellitus, 12 cases (9.8%) of myopia, 12 cases (9.8%) of congenital factors of choroid and 12 cases (9.8%) of ocular diseases were found. The frequency of severe impairment was adjusted in 25 (15 females, 10 males) out of 856 (501 females, 355 males) admitted to the department of ophthalmology during the first week of the study. 2. Ophthalmology: daily life and service status of the victim, family environment, oneself and the week, hope point, hope guidance, example, eye doctor, visual trainer and eye life training guidance, solution protocol, suitability, glasses, large book, shading, prescription and use training, simple walking guidance, white staff, life guidance, social protection system support and implementation of obstacles. The 47th Japan Society of Clinical Ophthalmology (October 1993) and the 4th Japan Society of Orthopedics (November 1993) were published in the Journal of Eye Movement and Education.
