项目摘要

昨年までの「障害を受け容れる」意味の概念分析とフィールドでの基礎調査をもとに、本年は、受障後に、障害を受け容れる諸段階に応じた下位概念を明確にすること、およびその諸段階での看護職からのケア(ケアリングの認知のありようを明らかにすることを目的とした。その目的に添って、患者の語りによる質的データを用い、初期からの体験に関する語りの変化を捉えるとともに、身体を通してみる患者自身の体験世界がどのように変容するのかをあわせてみるために、等間隔時系列に身体変化と患者の言動の記録を詳細に追っていく2つの方法でのデータ収集と分析を行った。これまでの基礎調査では、受障後の時期として回復期以降により着眼してきたが、この方法では、対象を、整形外科疾患、脳神経外科系疾患、神経内科系疾患に絞り込むことで同じような障害を体験した人がどのように異なる体験の違いを示すのかに焦点を当てた。また、質的研究方法の中でも、どの方法論が理論的枠組みとして適切かを検討し、最終的に、受障直後の急性期からの「障害を持つ」という体験の意味について、現象学的なアプローチをもちいて行うことを選択した。その結果、まず、障害を新たに受障した体験を語れる対象の選定、語る時期を、フィールドワークによって探索し、障害が異なったとしても受障後1週間を過ぎた頃から、参加観察と診療録から患者の言動が安定し、感情失禁のない患者でリハビリテーション訓練開始後の患者が適切であることがわかった。また、次段階では、この対象選定をもとに、本年は質的な患者の語りを5名の対象から得ることができ、さらに等間隔時系列奈身体の変化について、今後の研究の方向性を探った。5名の受障直後の体験は、現在、質的データに関するスーパービジョンを繰り返し受けているが、おおよその傾向として、初期の段階では「自分なりに出来事を解釈する」「状況の判断は困難であってもとにかくよい希望を持ち続ける」「仕方のないこととして何をすべきかを見いだす」などが体験からの意味として明らかになってきている。また、その初期の段階からの転帰は、生理学的に身体が回復し「動く」が変化することが契機となっており、わずかな変化がきっかけになって変化を推進していることがわかってきた。その一方で、残っていく身体の障害に対しては、「あきらめる」「あきらめの中にもよい希望を持ち続ける」という体験が繰り返し起こっていることが明らかになってきた。初期段階をすぎどのような「受け容れる」という体験の意味に変化するかを明らかにする、またケアリングの認知を具体的に語りの中から抽出することが今後の課題である。
