前年度はパルスオキシメーターのコンピューター処理とVTRの同時記録を可能にした。さらに重症心身障害児154名の歯科治療時のSpQ_2値を測定した。これらの最重度の心身障害児であっても、約9割は健常者と同様のSpsQ_2値を示すことを明らかにし、残り1割もSpQ_2値は90以上を示す事を明らかにした。本年度は153名の心身障害者について、種々の治療方法による歯科治療時の全身管理のモニタリングとしてパルスオキシメーター測定のVTRの同時記録を行った。心身障害者の全身麻酔を用いた集中歯科治療時の全身管理で、SpQ_2値の測定を61症例に行い、その結果、重度の脳性麻痺患者では覚醒時の自発呼吸が再開するまでに著しい遅延が見られることが明らかとなり、適切な抜管時期を知る上で、また病棟帰室後の呼吸管理にきわめて有用であることが明らかとなった。静脈内鎮静法を用いた場合の全身管理では42症例についてSpQ_2測定を行った。その結果、静脈内鎮静法の導入時および術中、術後に鎮静が深くなった場合の呼吸抑制を他のモニターより早期に知ることができ、より安全な鎮静法を行うことが可能となった。特に脳性麻痺患者の舌根沈下などを早期に知ることができ呼吸管理上きわめて有用であった。さらに通常の歯科治療時においても脳性麻痺患者36症例の測定を行い、頻回にSpQ_2値の著しい低下が認められ、呼吸管理上きわめて危険な状況にあることが明らかとなった。これらを回避するためにSpQ_2値を常時監視することが重要であることが明らかとなった。しかし不随意運動が著しい場合には測定不能となる場合も多く今後改善する必要があることが明らかとなった。コウシタ症例では笑気吸入鎮静法が不随意運動を抑制し、SpQ_2値を高い値に保つことができ、きわめて有用であった。また通常の歯科治療時にてんかん発作を起こした症例では呼吸状態を正確に把握でき適切な対処が可能となった。
