近年の遺伝子工学の急速な進展に伴い,リンパ球やマクロファージなど種々の免疫担当細胞が産生し,細胞間の情報伝達を行うサイトカインの働きがしだいに明らかになってきた.そこで本研究では,根尖性歯周炎罹患患者から病変部組織を採取してIL-1,IL-6,EGFおよび各種リンパ球の組織内局在を免疫組織化学的に調べるとともに,上記成分の活性の有無をELIZA法,bioassay法を用いて調べた.さらに根管内浸出液を採取して,病態の変化に応じたこれらの成分の経時的変化を同じくELIZA法,bioassay法を用いて調べた.1.被検者:根尖性歯周炎罹患患者[全身状態が良好で単根歯の根尖部にX線透過像を有し,外科的処理の対象となるもの(実験群),および通常の処置法で治療が可能なもの(対照群):各10名].2.試料の採取:臨床診査,X線診を行った後,各グループの被検者から,病変部組織および根管内浸出液を採取した.3.病変部組織の免疫組織化学的ならびにフローサイトメトリーによる解析:凍結切片を作成してIL-1,IL-6,EGFおよび各種リンパ球のモノクローナル抗体で染色し,各成分の組織内局在を調べるとともに,一部の試料についてはフローサイトメトリーで解析した.4.ELIZA法,bioassay法による病変部組織内および根管内浸出液のIL-1,IL-6,EGF活性の測定:ELIZA法では測定キットを,bioassay法ではマウス胸腺細胞を用いて解析した.その結果,対照群に比べ実験群では,とくに病変部組織においてIL-1,IL-6活性の上昇を示唆する結果が得られたが,根管内浸出液に関しては,現時点では有意の差を見出せなかった.さらに症例を増やし検討を進めている.

In recent years, rapid progress has been made in genetic engineering, and the production of immune-responsible cells and the transmission of information between cells have been carried out. In this study, IL-1, IL-6, EGF and various proteins were detected in the tissues of patients with apical periodontitis. The root canal leaching solution was collected and pathologically changed. The components of the root canal were changed at the same time. ELIZA method and bioassay method were used to adjust the root canal leaching solution. 1. Patients suffering from apical periodontitis [general condition: good, X-ray transmission image of apical part of single tooth, surgical treatment: possible (group), general treatment: possible (group): 10 each].2. Sample selection: Clinical examination, X-ray examination, each group of patients, diseased tissue and root canal extract were taken. 3. Immunohistochemical analysis of diseased tissue: Frozen sections were prepared to detect IL-1, IL-6, EGF and various kinds of antibodies, and the tissue structure of each component was adjusted. Some samples were analyzed by ELIZA method. Bioassay method for determination of IL-1, IL-6, EGF activity in root canal extract and diseased tissue: ELIZA method for determination of IL-1, IL-6, EGF activity in root canal extract and thymocytes. The results showed that the activity of IL-1 and IL-6 in root canal extracts increased significantly in the root canal tissues. Today's symptoms are increasing and discussions are progressing.
