褥瘡の形成は、局所に循環障害を起こす圧迫と時間が関わっており、仰臥位の保持は普通2時間を安全の限界として、臨床の場では毎2時間の体位変換が行われている。江幡らは褥瘡の形成の要因を解明すべく、すでに成人,老人女子を対象に、褥瘡好発部位の体圧分布状況の解明を行うと共に、褥瘡予防具(特にエアマット)による除圧,除湿の効果を検証してきた。毎2時間の体位変換が局所の循環障害とどのように関わっているかを知るためには、更に体圧と皮膚血流との関連、すなわち長時間仰臥位を保持した際の仙骨部の阻血状況を知る必要があると考えた。今回、成人女子6名を対象に、レーザー血流計(アドバンスALF21D)と体圧分布測定装置を用いて、仙骨部とヤコビー線と正中線交差部(比較的圧のかからない部位)における、(1)腹臥位時の皮膚血流と、(2)仰臥位2時間保持及びその後左側臥位に体位変換し30分間保持した場合の体圧と皮膚血流との関連を標準ベッド及びエアマット(サンケンギャジマット)を用いて、経時的に同時自動測定をした。結果、(1)体位別,部位別にみた仙骨部とヤコビーの血流量は、腹臥位での1名を除き、いずれも仙骨部が多く仰臥位が多かった。(2)2時間30分後の体温,皮膚温は共に上昇した。(3)仰臥位2時間の血流量と変動の状態は、仙骨部は増加傾向を示し、仙骨部,ヤコビー共に2時間後の減少は見られなかった。(4)体位変換後(左側臥位)の血流量は、いずれも瞬時に増加し、以後の減少傾向は個人差が大であり、30分経過後では臥床開始時と同一にはならなかった。(5)仙骨部体圧と皮膚血流との関連は顕著にはみられず、ベッドに比しエアマットは明らかに除圧し皮膚血流量は増加傾向を示した。以上の結果を得たが、今回は例数が少ないため若干の傾向のみが示唆されたが、体圧と皮膚血流の同時測定方法について検討の余地があり、測定方法の確立を早急に目指したい。

(5)The relationship between body pressure and skin blood flow is not obvious, but the relationship between body pressure and skin blood flow is obvious The above results were obtained, and the number of cases was reduced. Some tendencies were indicated. There was room for discussion in the simultaneous measurement method of body pressure and skin blood flow. The establishment of the measurement method was urgent.
