褥瘡予防の効果的な、かつ効率の良い適切な看護を提供するために、局所の圧迫の程度や、体圧の分布状況を知るのみでなく、褥瘡好発部位における循環(皮膚血流)状態との関連について検証することは重要な課題と考える。本研究では、健常な老人64才〜79才の男性8名,女性8名を対象として、褥瘡好発部位(仙骨部,大転子部)の体圧と皮膚血流量の関連性を究明するを目的とした。実験方法:すでに報告した「体圧分布測定装置」に、レーザー血流計(アドバンス社製,血流プローブは外径10mm,厚さ3mm円盤形)2個を導入し、圧力センサ,温度センサを組入れて測定した。寝具には、基準ベッド(スプリングマットレス,マトレスパット),噴気型エアマット(SGM-1,三和化研工業製)を用いた。測定体位は仰臥位とし、仙骨部と左側臥位における大転子部の2カ所について測定した。ベッドおよびエアマット上において各々仰臥位,左側臥位,仰臥位の順に体位変換し、体位の保持は各10分間で一人あたりの所用時間は1時間とした。結果:(1)局所にかかる圧力は高ければ高いほど除圧後の皮膚血流量の増加が大きい。(2)体位変換直後の皮膚血流量は一時的に増加した。(3)体圧の男女差は、骨盤と仙骨の形状の差異により、女性の方が高い傾向であった。(4)寝具別では、ベッドはエアマットより加圧中の皮膚血流量が少なく、体位変換後の皮膚血流量の増加は大であった。このことは、除圧後の血流量の増加量が寝具による加圧の高低と関連することを示唆した。(5)痩せ型の方が肥満型より体圧が高い。(6)大転子部の方が仙骨部より体圧は高く、皮膚血流量は少ない。(7)皮膚温と皮膚血流量との間には明確な相関はみられなかった。
