随意に排便がコントロール出来ない人工肛門に対して、皮膚と腹壁のストーマの貫通をわずかにずらすことにより皮膚がストーマの弁となって畜便が出来、排便の際には円筒形のチューブを挿入してストーマを直線化することにより排便が可能であるという、新しいスライディングストーマを考案し、構造上の検討と作成後の経過を中心にイヌの実験的で検討した。われわれ考えた新しい人工肛門(スライディングストーマ)の構造は人工肛門の腸管が腹壁と皮膚を貫通する部分でずらすことにより、皮膚および皮下で排便がコントロールされる。排便のさいは、スライディングチューブを挿入することにより皮膚と腹壁の腸管が直線化され排便が可能となる。実験結果1.12頭のイヌにおいてスライディングストーマを作成した。2.12頭中3頭に作成後、ストーマに関連するトラブルが起こった。3.12例中2頭が2週間以内に死亡した。このうち1例が排便が見られなかった。4.最長2ヵ月間、術後経過をみた。5.12頭中4例において経過良好で排便状態が観察できた。6.これらの4頭はスライディングチューブを挿入し、固定したまま排便を行わせた。考察個体差により直線化、あるいは皮膚による密閉が一定化しなかった。手術手技的にさらに検討の必要がある。便の正常によりチューブ内に便が詰まることもあり、スライディングチューブの円筒径など構造上の検討の必要がある。排便に良好なスライディングチューブを開発すべきである。腹部単純写真で便が貯留しているものがあり、これらはスライディングストーマにかかわらずあるのかどうか不明であり、今後の検討を要する。スライディグストーマの臨床応用の可能性はあるが、今後さらに基礎実験が必要である。スライディングストーマが長期の経過後の変化など観察する必要がある。
