我々は、以下の2点に関し平成6年度後期に配分を受けた科学研究費補助金により研究を実施した。第一に、診療構造を把握し在院日数を短縮するための手法として、システム開発・管理に用いられているネットワーク・プランニングの手法が応用可能であるかを検討した。まず、1991年度に本学付属病院呼吸器内科で肺癌の診療を受けた72名の患者より典型的な診療経過をたどったと考えられる5例をモデルケースとして選び、肺癌診療過程に対し重要な影響を与えたと考えられる検査・事象・治療として、入院予約、入院会議、入院、各検査の予約・提出及び報告、CT検査予定会議、ステージングなど15項目を抽出・設定した。これら各検査・事象・治療について、相互の情報の流れおよび前後関係の決定をおこない、肺癌患者の標準診療PERT(Program Evaluation and Review Technique)モデルを作成した。このモデルにより、本院での肺癌診療過程で診断までに要する期間に大きな影響をおよぼす過程(クリティカル・パス)は、入院予約から入院までと、気管支鏡検査およびこれに引続き行われる病理検査であるとの推定が可能となった。現在に至るまで、特定の疾患の診療過程についてPERTモデルを作成した報告はなく、今回の研究により、ネットワークプランニングの手法が診療構造の把握に応用可能であることが示された。この推定を検証するため、第2の研究項目として1991年度に本学付属病院呼吸器内科で肺癌の診療を受けた72名の患者の診療情報を抽出し、現在、PERT手法を応用して、診療過程の構造を解析をおこなっている。

The following two points are related to the allocation of scientific research grants in the late part of the sixth year.
