本研究は定員100名の特別養護老人ホームをフィールドとして行った。入所者本人と介護者に研究の主旨を説明し、双方もしくは本人との意思疎通が困難であった場合で介護者の同意を得られたのは65名であったため、この65名を本研究の対象者とした。歯科疾患リスク診査と日常生活動作能力により対象者の口腔衛生状態を1.歯および義歯の清掃に介助が必要でない(現状維持群)、2.歯科治療は必要でないが、一部介助もしくは全介助による口腔衛生管理が必要(口腔ケア群)、3.早期に歯科治療が必要(要治療群)の3群に分類し、それぞれ12・49・22名であった。なお、要治療群22名中18名が口腔衛生管理も併せて必要と評価された。さらに現場維持群12名中口腔衛生状態を自分自身で管理している人は4名で、他の8名は歯も義歯も無く、特別な口腔衛生管理を施さなくても口腔衛生状態は良好であった。口腔ケア群において歯の衛生管理が必要とされた人は36名(要歯のケア)、義歯の衛生管理が必要とされた人は25名(要義歯ケア)であった。なお口腔衛生管理は、毎食後の洗口とブラッシングを義務づけ、1週に一度歯科医師がその効果をQIインデックスを用いて判定した。入所者の生活や行動の変化は、介護職員が記録する介護日誌と看護職員のコメントにより評価し、口腔衛生管理の受容の判定を歯科医師と共に判定した。本研究の主旨である、口腔衛生を維持するための行動が及ぼす影響が顕著であったのは、口腔ケタ群と判断された人達であった。特に『要歯のケア』36名中口腔衛生管理を受容し、生活や行動に変化が無かった人は26名で、他の10名はこれを拒否するか行動に変化が現われた。一方『要義歯ケア』25名中21名は口腔衛生管理を受容し、4名に行動の変化が現われた。行動の変化で最も特徴的なことは、口腔衛生管理担当介護職員や歯科医師を避ける・無視する、そして口を開かない・きかないであり、これは痴呆の程度が軽度・中等度の人に多く見られた。痴呆が重度である人は、口を開くことを拒否したとしても、口腔衛生管理担当者を拒否する姿勢は見られないケースが多かった。
