標記研究テーマの下で設定した具体的課題と得られた知見は以下の通りである。1.在宅重障児の実態調査:訪問教育、通学、両者の中間的タイプ(訪問教育の枠内でのスクーリング)の三形態で指導をうけている者計8名を対象に実施した。原因に関しては5例がいわゆる先天性脳障害であったが、3例が交通事故、溺水による者で、近年の傾向を反映している。人口呼吸器使用2名、栄養補給は経(鼻)管3名、胃ろう2名など医療的管理の必要度が高い。ADLは全面介助を要し、睡眠・覚醒リズムは2名が未確立である。2.発達神経心理学的評価:脳および各種感覚系の基礎機能を脳波誘発反応測定によって評価を試みた結果、脳幹水準へ及ぶ重篤な脳機能障害が指摘された。1例に視覚障害も認められた。3.指導場面における反応測定と評価:指導場面での教師と対象児のやりとりを、映像・音声記録とともに、対象児の心拍反応を無拘束計測し、同一時系列上で両者を詳細に捕捉、評価した。その結果、心拍反応上の発達的特徴から、(1)無反応・防御的加速反応優勢(3名)、(2)定位的減速反応優勢(3名)、(3)能動的加速反応優勢(2名)の3群に分類され、これと担当教師の発達評価はほぼ一致した。しかし、特に(1)、(2)群では、肉眼観察しうる行動的変化は極めて微細なため、指導中にこれを的確に把握できない場面も多かった。4.まとめ:医療的管理を要する在宅重障時の増加傾向が指摘される。訪問教育は当初心配されたほどにはこれらの対象児に負担をかけるものではなく、むしろ定期的指導によって生活リズムの確立をはじめとする多面的な効果が期待される。特に、可能な範囲でスクーリングを実施する意義が大きい。一方、指導内容・方法に関してはなお課題が多く、生理心理的情報の即時フィードバックの試みなどを進め、今後さらに検討する必要がある。
