本研究課題の最終目標は、今後の国内の肺炎球菌ワクチン政策に寄与するために、国内における肺炎球菌性肺炎固有のQOL値および国内の最新の肺炎球菌血清型分布を用い、高齢者における肺炎球菌ワクチン費用効果分析を行うことである。令和4年度においては、令和3年度から継続し、最新の国内の肺炎球菌性肺炎における、肺炎球菌血清型分布の評価した。2019年4月以降に全国の23の医療機関で治療を行った肺炎球菌性肺炎における、肺炎球菌分離株および患者情報を回収した。合計537症例の肺炎球菌性肺炎が登録され、うち313例はすでに血清型が決定している。現在、未検査分の血清型判定および患者情報について統計情報をまとめている。登録された症例は、2019年から2021年7月までの肺炎球菌性肺炎の症例であり、新型コロナウイルス感染症の流行前後の期間が含まれている。新型コロナウイルス感染症流行後、人々の基本的感染症予防、人と人の接触機会が減少したことにより、様々な感染症の発生数の減少が指摘されているが、肺炎球菌感染症も同様に発生数が減少していると考えられている。今回の結果を使用し、新型コロナウイルス感染症による肺炎球菌感染症の影響も併せて評価できると考えている。肺炎球菌ワクチンは令和4年度にPCV15/20が薬事承認され、PCV21の開発も進んでいる。このように現行のPPSV23/PCV13に加え、新たな肺炎球菌ワクチンが使用可能となるにあたり、令和5年度には、最新の国内の結果を利用して、新たな肺炎球菌ワクチンも含めて、高齢者における肺炎球菌ワクチンの費用効果分析を行う予定としている。
