本研究は特発性正常圧水頭症の運動機能障害を評価・観察する方法を明らかにするものである。日常生活において頻度の高い複数の行動をバイオメカニクスの手法を用いて評価し、他疾患からの正確な鑑別、従来評価できなかった重症者の治療効果をより客観的にとらえ、診断精度を高めることを目的とした。具体的には１）３次元動作解析を行いた特発性正常圧水頭症特有の動作障害を特定。２）３次元動作解析を用いた髄液排出試験およびシャント術の効果判定および信頼性・妥当性の検討。３）動作障害を起こす病変部位の特定および病態の解明をめざした。平成２６年度には研究計画の倫理委員会への提出を行い、受理後に特発性正常圧水頭症患者および健常対照群のエントリーを行った。また上記患者群および健常者群における登録用データベースを作成した。神経心理検査・排尿日誌の記録、歩行検査および脳МＲＩ撮影を行い、データベースに入力を開始した。髄液排出試験前後での３次元動作解析を行い、特発性正常圧水頭症の歩行特性を把握を試みた。＜結果＞特発性正常圧水頭症の疑いで入院した症例において髄液排出試験前後での詳細な３次元動作解析は手術適応を決定するうえで有用である可能性が示唆された。今後得られたデータからひきつづき解析を行っていく予定である。＜３次元動作解析＞複数のカメラで撮影した映像をデジタル化し、マーカー座標に基づく動作分析、身体重心、仮想点の算出などを行う。
