Voluntary breath hold technique is a simple method to control the respiration-related motion of the tumor during irradiation.In 2003, we have developed a new respiratory monitoring device for stereotactic body irradiation using both a cyberknife and linac machine. The system consists of a small laser pointer on the abdominal wall and an overhead board (see figure). A laser beam projected on the board above a patient's face moves according to the respiratory cycle, and correlates well with the motion of the chest wall. Patients can control the motion of the chest wall because they can recognize the motion of the laser beam on the board. A therapist can monitor the motion of the laser beam on the board through a room camera.Five normal volunteers were tested. A machine vision system (XV-1000, KEYENCE, Japan) was used to evaluate the chest and abdominal wall position in five sessions of 15-second deep-inspiration breath hold (DIBH) with or without thisdevice. The change in the position of the chest and abdominal wall with or without this device during 5 repetitions of 15-second breath-holding (BH) was measured at 80 msec intervals under 3 respiration BH types : shallow expiration BH, shallow inspiration BH, and deep inspiration BH.Using voluntary BH technique without any monitoring devices,1)the deviation of the wall position is smallest in shallow expiration BH.2)shallow inspiration BH and deep inspiration BH may not be suitable for radiotherapy, because the deviation of the wall position is large.Using voluntary BH technique with a monitoring device which displays the wall position to the patient,1)the deviation of the wall position is small enough in shallow expiration BH and shallow inspiration BH.2)the deviation is still large in deep inspiration BH.Patients may perform breath-hold maneuver in a highly reproducible fashion using a new respiratory monitoring device, particularly in shallow expiration BH and shallow inspiration BH.
