本研究は、健康危機により医療資源が大きく不足する状況での資源配分のあり方を提案することを目的として、事例や法令等の分析、保健医療・福祉サービスの水準の変化の健康影響の分析、資源配分やサービス水準の調整にかかる専門家の見解の集約、地域住民を対象とした公的討議などを実施するものである。1年目の主な実績は下記の通り。1.国内外の事例収集・分析による課題の整理と類型化：わが国および同等の保健医療水準を有する主要先進国として、主に米国、英国、EU加盟国、イスラエルについて文献調査および関係機関へのヒアリングを行った。特に、米国のCrisis Stanadards of Care（CSC）の適用状況やこれに関連する議論について、学術論文や公的機関による指針等をレビューした。米国ではバイオテロや新型インフルエンザを契機にCSCのガイドラインが策定され、倫理的基盤、地域社会の関与、法的要件の確認、明確な実施基準、エビデンスに基づいた診療・対応、などが主な要素とされている。COVID-19のパンデミック期において、約半数の州は州としてのガイドラインを策定していた。また、日本での導入にあたっては、明確な基準設定のための情報集約、オールハザード対応の危機管理体制、医療専門職等の法的な扱い、トリアージに関する責任のあり方などが論点になると考えられた。2.保健医療・福祉サービスの提供体制の変化と集団の健康アウトカムとの関連の分析：COVID-19の流行拡大期における医療機関の受診動向を診療報酬明細書（レセプト）の情報などをもとに分析し、流行拡大期に受診が減少することと、減少の大きさや回復の速さなどは診療科によって大きく異なることなどが明らかになった。
