The aim of this study was to establish the effective and harmless way of exercise therapy of the disabilities, especially of the obese patients, and to investigate metabolic changes of fat and carbohydrates of those.Participants of this study were patients admitted in our hospital for past 2 years, from 17 to 66 years old, and female/male was 6/4. Base line body mass index were 35.7 to 49.2 and they had diabetes mellitus except one and half of them suffered from hypertension, sleep apnea syndrome and dyslipidemia.Before exercise therapy, we tried treadmill exercise test, checked the risk of coronary heart disease and fatal arrhythima and so on. Then exercise therapy was done for 30-40 minutes at observed condition under heart rate of anaerobic threshold from the treadmill test, 4 or 5 times per week.Diet therapy was done with the exercise therapy. At first, calorie intake was 25-27Cal/day/idealbody weight and gradually reduced to 800-1000Cal/day.With these therapies, we lectured the actual regimen of exercise and diet therapy and lifestyle modification program, the risk of obesity.By these therapy, among younger adults, body weight loss of 5-9kg/month was obtained and 2-3kg/month among elder adults without harmful accidents. But lean body mass were not reduced each, so it was supposed the loss of body weight was that of fat tissue. By these fat tissue loss, insulin resistances of diabetic patients were reduced. Among with hypertension, decrease of anti-hypertensive agents were brought and improvement of dyslipidemia were seen. In short, this study revealed the loss of fat tissue lead to the improvement of Metabolic syndrome and prevention of coronary heart disease in Japanese. Further more rebound phenomenon of body weight were not seen after discharge.We established the effective and harmless effective therapy based on treadmill test and education.
本研究的目的是建立一种对残疾人,特别是肥胖症患者有效而无害的运动疗法,并探讨肥胖症患者脂肪和碳水化合物代谢的变化,研究对象为我院近2年来收治的17 ~ 66岁残疾人,男女比例为6/4。基线体重指数35.7 ~ 49.2,除1/2合并高血压、睡眠呼吸暂停综合征、血脂异常外,其余均合并糖尿病。检查冠心病和致命性心脏病的风险等。然后进行运动疗法30-在平板试验无氧阈心率下观察40 min,每周4 ~ 5次,运动疗法配合饮食疗法。开始时,热量摄入量为25- 27卡/天/理想体重,逐渐减少到800- 1000卡/天。通过这些治疗,我们讲授了运动和饮食治疗的实际方案和生活方式改变计划,肥胖的风险。通过这些治疗,年轻人体重减轻5- 9公斤/月,老年人体重减轻2- 3公斤/月,无有害事故。但是瘦体重并没有减少,所以体重的减少被认为是脂肪组织的减少。通过这些脂肪组织损失,糖尿病患者的胰岛素抵抗降低。在高血压患者中,降压药物的使用减少,血脂异常改善。总之,这项研究揭示了脂肪组织的损失导致日本人代谢综合征的改善和冠心病的预防。出院后未出现体重反弹现象,在活动平板试验和教育的基础上,建立了有效、无害的治疗方法。
