Changes in "residence" of the Frail Elderly : a cohort study

本研究的目的是跟踪自引入Kaigo Hoken（长期护理保险）之前就存在的队列组，从而在区域范围内澄清体弱老年人如何在导致死亡的护理需求程度发生变化的过程中移动他们的"住所"。本研究的特点是，考虑到老年人及其照顾者的社会心理和社会经济决定因素，对影响居住地迁移的因素进行了综合分析。研究内容和方法（1）健康老年人出现需要照顾的状况和居住地迁移的队列研究：我们利用受访者的数据，对65岁或以上的老年人进行了一次完整的点算。对于截至2000年4月1日尚未申请海后法证的3 295人，跟踪了他们到2004年9月的情况变化。（2）关于脆弱长者住所迁移的同期研究 关于我们 rly：至于截至2000年4月已获发长期护理需要证明书的458名体弱长者，我们会跟进他们的情况变化，直至2004年9月为止。（3）与居住地迁移相关的照顾者和地区特征研究主要结果和未来研究目标经过4.5年的追踪，当时健康老年人的情况如下："死亡" 368人（11.2%），"迁出城镇" 69人（2.1%），"居住在城镇" 2,858人。在此期间，439人（13.3%）获得长期护理需要证明，细分如下："留在家里"; 283人;"其他设施"; 81人;和"返回留在家里（家→设施→家）"; 75人。作为影响体弱老人迁移居住地的因素，老人的外出频率和居住环境被提取出来，照顾者的继续居家护理意愿被提取出来。今后，我们的目标是将照顾者和地区特征的综合因素分析扩展到2002年的调查结果。

The purpose of this study is to track cohort groups existing since before the introduction of Kaigo-Hoken (long-term care insurance), and thereby clarify on a regional scale how the Frail Elderly move their "residence", in the course of a changing degree of the need of nursing care leading to death. The feature of this study is that we have comprehensively analyzed the factors affecting the movement of residence, in consideration of the psychosocial and socioeconomic determinants for elderly people and their caretakers.Contents and Method of the Study(1)Cohort study on the emergence of conditions in need of care for healthy elderly people, and the movement of residence : We used the data of the respondents for a complete enumeration on elderly people at the age of 65 or more. During this period, 439 persons (13.3%) received the certification of long-term care need and the breakdown was as follows : "staying-at-home" ; 283 persons, "other facilities" ; 81 persons, and "return to staying-at-home (home→facilities→home)" ; 75 persons. As the factors involved in the movement of the residence of Frail Elderly, outing frequency and living environment were picked up for elderly people, and the degree of strong will to continue nursing at home was picked up for caregivers.In the future, we aim to extend a comprehensive factor analysis including caretakers and regional characteristics to the survey results obtained in 2002 Less
