本研究は、訪問看護師がチームメンバーと合意形成にむけて、どのようなアプローチを行っているかを明らかにし、「在宅移行期におけるチームメンバーとの合意形成にむけたガイドライン」を開発することを目的としている。平成27年度は、在宅移行期における合意形成の必要な状況を明らかにするという目標を立て、インタビューを実施するための準備期間とした。インタビューガイド作成にあたり、在宅移行支援、地域包括ケアシステムに関する最新の動向と課題を整理するため、学会等への参加と文献等を購入し、文献検討を行った。その結果、地域包括ケアシステムの構築における在宅移行支援の重要性や在宅移行支援によるチームメンバーとの協働の重要性は着目されているものの、具体的にどのようにアプローチしていくかまでは明らかにされていないことがわかった。また、在宅移行支援、地域包括ケアシステム、合意形成に関する文献をもとにインタビューガイドを作成した。文献をもとに、用語の定義の見直しや在宅移行期における訪問看護師によるチームメンバーとの合意形成の必要な状況、合意形成にむけたアプローチについて対象者が豊かに語れるようインタビューガイドの洗練化等を行った。所属機関の研究倫理審査委員会の承認を得て、訪問看護ステーション責任者に、研究の目的、方法、質問内容、倫理的配慮および対象者の紹介依頼などの内容を盛り込んだ文書を作成し、訪問看護の経験が3年以上であり、病院スタッフや在宅スタッフとともに在宅移行支援に関わった経験があり、合意形成にむけたアプローチの成功体験がある看護師の紹介を依頼した。研究者の退職に伴い本研究は廃止となるが、今後はさらに対象者確保のためのアクセスが必要と考える。
