本研究は糖代謝異常を有する患者を対象に、生体における予備能と考えられる「副交感神経活動リザーブ」を積極的看護介入によって高めることにより、糖尿病性神経障害を予防・改善させることを目的としていた。既に本研究に至るまでの基礎研究において、糖尿病性神経障害の症状がみられる前から糖代謝異常が重症であるほど副交感神経活動リザーブは低いことを明らかにしてきた（後藤ら 2014）。本研究の具体的な研究項目は、「①糖代謝異常と副交感神経活動リザーブの関連についてさらに検討すること、②副交感神経活動リザーブを高める看護介入方法として継続的にゆっくり呼吸するSlow Breathing Exercise(SBE)を4週間実施することにより、糖尿病性神経障害が予防・改善されるか否か検討すること」であった。平成28年度の研究実績としては、本研究の実施について研究代表者の所属機関の倫理審査委員会にて承認を得た。また、所属学会学術集会の交流セッションにて予備的検討のデータを提示しフロアと意見交換を行った。この時に提示したものは、糖尿病性腎症に罹患している患者1名にSBEによる看護介入を行った結果、SBE前後に副交感神経活動指標と末梢循環指標の平均値が増加し、呼吸数と交感神経活動指標、知覚閾値の平均値が減少したデータであった。この予備的検討を通し、得られた結果はSBEによる副交感神経活動リザーブの増大によってもたらされた可能性があると解釈できた。

This study aims to prevent and improve diabetic neurological disorders by actively nursing intervention in patients with abnormal glucose metabolism. In this study, we conducted basic research on diabetic neuropathy, and found that the symptoms of diabetic neuropathy were abnormal in glucose metabolism and severe in parasympathetic nervous activity (Goto, 2014). The specific research items of this study are: (1) Investigation of the relationship between abnormal glucose metabolism and parasympathetic activity;(2) Nursing intervention for parasympathetic activity;(3) Prevention and improvement of diabetic neurological impairment; and (4) Implementation of Slow Breathing Exercise(SBE). The results of the study conducted in 2008 were recognized by the Ethics Review Committee of the institution to which the representative of the study belongs. The exchange of ideas at academic meetings of affiliated societies is carried out in preparation for discussion and prompt. The results of nursing intervention showed that the average values of parasympathetic nerve activity index and peripheral circulation index increased before and after SBE, and the average values of respiratory sympathetic nerve activity index and cognitive threshold decreased. The result of this study is that SBE is the most important factor in the development of brain function.
