项目摘要

2022年度は海外論文の文献検討を実施した。精神障害者の肥満及び糖尿病への地域ケアについて、看護研究は日本で報告されておらず、海外知見の収集が必要であった。そのなかでレビュー文献より、専門家が立案した健康介入プログラムについて改善効果がほとんど見出されないという知見があり、精神障害者の肥満および糖尿病は看護にとって困難な課題であることが改めて明らかになった。この理由についてさらに文献を分析し、研究計画に反映してゆく必要がある。精神障害者の身体合併症の問題へ取り組みについて、世界的な流れとしては、健康介入プログラムの立案に当事者の視点が必要であるとされている。本研究計画における当事者参加の意義について確認できた。今後は文献検討を続け、学会誌等で報告することを目指している。22年度は精神科訪問看護実践経験の豊富な研究分担者を追加した。臨床的な視点が増補されることで、より良い研究成果が期待できると考えている。研究計画の中で、質問紙の作成に取り組んできた。肥満や糖尿病は健康問題として頻度が高いが、精神疾患患者への訪問看護実践のなかでは中心的な課題ではない。情報量および回収率を確保するための方略が必要である。また先行研究として取り組んでいる身体疾患を併せ持つ精神疾患患者の訪問看護サービスの実態と課題（18K10327）のデータ分析から、社会背景の中で訪問看護師が置かれている状況と課題が明らかになり、今後の質問紙作成への示唆を得た。
