项目摘要

本研究は、市区町村主体で行われる出産前後及び乳児期の全母子を対象とする母子保健行政サービスと、母子健康指標との関連を明らかにすることを目的する。本研究の初年度である2022年度は、まずはじめに、日本における母子の健康指標の明確化を行った。従来、母子保健のアウトカム指標では死亡指標が用いられることが多い。しかし、日本の母子保健における死亡指標は、世界有数の低率国である。そこで、国内外の文献レビューを行い、本研究における母子の健康指標の定義を行うための知見の整理を行った。その結果、日本は、他の先進諸国と比べて低出生体重児割合が高いまま推移しており、多岐にわたる要因に関する報告は多数なされているものの、プロセスとの関連をみた先行研究は少ないことがわかった。また、母子保健対策の重要課題とされる産後うつは、周囲から気づかれにくく、医療に適切に繋がりにくいことから、産後の家庭訪問効果が期待されるものの、国内研究においては定量的評価は報告されていなかった。以上を踏まえて、本研究では、低出生体重児割合及び産後うつの２つの健康指標に着眼することとした。次に、市区町村における母子保健行政サービスの把握のため、研究班で市区町村対象の質問紙調査票の内容の検討を行った。その後、現任保健師を交えて検討し内容の妥当性を確認した。さらに、市区町村単位の低出生体重児数は、公的統計ミクロデータに該当するため、利用申請に向けた事前相談を行った。

The purpose of this study is to clarify the relationship between maternal and child health indicators and administrative services before and after birth and during lactation in urban and rural areas. In the first year of this study, the maternal and child health indicators were clarified in 2022. The indicators of maternal and child health care are used in many ways. Japan's maternal and child health indicators are among the world's lowest. The definition of maternal and child health indicators in this study was reviewed. The results show that Japan and other advanced countries have higher birth weight than other countries. Most of the reports on the main causes of low birth weight are related to the study of low birth weight. The important topic of maternal and child health care policy is the evaluation report of postpartum, weekly and appropriate medical treatment, postpartum family visit and expectation, domestic research and quantitative evaluation. This study focuses on the health indicators of low birth weight and postpartum. Second, urban and rural maternal and child health administrative services to grasp the content of the study class urban and rural questionnaire to discuss After the appointment, the current health care provider will confirm the appropriateness of the content. The number of low birth weights in urban and rural areas is estimated by statistical methods.
