肺移植は末期呼吸不全患者に対する唯一の救命手段だが、他の臓器移植に比べて長期予後は明らかに不良である。長期予後改善のために克服すべきは、慢性拒絶が主因の「慢性期移植肺機能不全(chronic lung allograft dysfunction: CLAD)」で、新たな治療法の確立が必要である。肺移植臨床を反映するCLADの実験モデルは乏しい現状をふまえ, 本研究の目的は、①臨床状況を反映するマウス肺移植CLADモデルを開発すること、②そのモデルを用いた慢性拒絶に至らしめる免疫応答の解明と新たな治療ターゲットの提案を目指すこと、とした。BALB/cマウスの左肺をB6マウスに同所性に移植し、術後に低用量の免疫抑制(ステロイド・シクロスポリン)を行うことで、移植肺の組織学な所見として移植後約2週間で急性細胞性拒絶反応が、移植後約1か月でリンパ球の集簇とともに血管気管支周囲や胸膜に線維化が生じることを確認した。研究目的①が臨床状況を反映するマウス肺移植CLADモデルの確立であったため、開発したマウス肺移植モデルの移植後変化から線維化に至る過程が肺移植における臨床状況を反映するかを検討するため、臨床肺移植後症例の血液検体を採取し、サイトカイン・ケモカイン等を解析することで、本マウス肺移植モデルの移植後経過と比較検討することや、マウス肺移植モデルを用いた研究が行われている他の研究施設の視察や意見交換等を計画したが、COVID-19感染蔓延下で肺移植症例数が減った時期があったことや施設間交流の制限があったことから、本年度の研究費交付から2022年度末まで研究を進めることが困難であった。
