项目摘要

本研究は日本人の死因の過半数を占める悪性新生物（がん）、心疾患、脳血管疾患などの生活習慣病に関する傷病名を対象とし、それらの生活習慣病に関する傷病名の電子レセプト情報を疾病登録データと照合し、電子レセプト情報上の特徴明らかにすることで、電子レセプト情報の各項目の組み合わせと電子レセプト上の傷病名の確率を明らかとする確率計算式を開発するものである。今年度は昨年抽出した電子レセプト情報を院内がん登録データと紐づけし、がん患者と非がん患者の照合を行った。電子レセプト情報のうち「がん」と病名のあるもののから、（1）レセプト病名（2）抗がん剤・内服薬（商品名）（3）抗がん剤・点滴薬（商品名）（4）処置（抗がん剤胸腔内注入など（5）手術の術式名（6）腫瘍マーカーの各項目における事後確率を算出し、ベイズの展開公式により各事後確率を統合して、確率計算式を開発した。今年度は2021～2022年の院内がん登録データや放射線部の心臓カテーテル検査実施記録データが完成し次第モデルに投入して予測（判別）の精度を向上させる（ベイズ更新）。最終的には自動的に生活習慣病症例であるか否かを判別する機能（Native Baysian filtering）を設け、カットオフ値の検討はこの段階で行い、確率計算式を完成させる予定である。さらに、本研究課題を学会等で報告し、その際に指摘された箇所と明らかになった問題点を修正・改善する。

This study was conducted to identify the most common causes of death in Japan, including sexual novelties, heart diseases, vascular diseases, and lifestyle diseases. The combination of various items of electronic information and the accuracy of the name on the electronic information are clearly defined. This year's e-mail address was sent to the hospital for registration, patient registration, and patient registration. The electronic information is divided into three parts: (1) the name of the disease;(2) the name of the drug;(3) the name of the drug;(4) the treatment;(5) the name of the surgical method;(6) the calculation of the post-accuracy rate of each item of the tumor;(7) the development formula;(8) the integration of the post-accuracy rate; and (9) the development of the calculation formula of the accuracy rate. This year, from 2021 to 2022, the accuracy of the hospital's registration and radiation department's heart test records has been improved (updated). The final function of the automatic lifestyle disease is set, the value of the discussion is set, and the accuracy calculation formula is completed. This research topic is reported by the Institute of Science and Technology, and it is pointed out that the problem points are corrected and improved.
