项目摘要

2019年の新規導入透析患者の平均年齢は69.20歳であり、前年と比べても0.16歳高齢化している。また患者のQOLや生存率の向上、医療費削減、地域包括ケアシステムの政策も相まって、在宅血液透析（HHD）の普及が急がれている。また、COVID -19に伴う医療危機を踏まえると、HHDの定着が必然と迫っている。HHD実施に向けては患者及び介助者が自己管理できるよう教育・管理が必要となるため、その実施の可否は患者及び介助者が自己管理できるよう教育・管理できるか否かが要となる。このような現状下で、わが国の在宅血液透析患者数は、全透析患者の0.2％にとどまり、高齢化も進んでいる。また、在宅血液透析関連診療報酬の規定に基づき「在宅血液透析（HHD）管理マニュアル」が作成されているが、HHD導入教育から指導管理体制や継続的な支援が必要なため利活用は進んでいない。本研究ではHHD普及を目指し、在宅患者宅と施設とを利用者限定のWebで接続し、両者で情報共有しながら、多職種医療従事者が患者の医療情報を地域で共有できる模擬システムを構築する。次に、模擬高齢者HHD患者のアセスメントができる自己管理支援システムを本研究者らの既存の地域連携型透析患者支援システムに機能追加として構築する。また、本研究では高齢者HHD普及を促進し、予期せぬ事態を減らして効率の良い療養経過を継続するために、利用者限定のWeb接続とし、多職種医療従事者が地域で患者情報を共有でき、高齢HHD 患者が地域で安心して治療・自己管理できる地域連携型透析患者支援システムを構築する。本システムは機能追加で構築可能なため、短期間・低設備投資で実現できる。
