文科学研究費補助金により,電子セクタ-型経食道超音波探触子を購入でき,それにより研究課題の一部を終了することができた。1).高脂血症患者,特に家族性高コレステロ-ル血症患者を対象として,胸部大動脈の動脈硬化病変を診断し,その血管特性についても詳細に分析した。家族性高コレステロ-ル血症(FH)患者17名を対象とし,同年齢の健常者21名と,胸部大動脈の動脈硬化症変の発症頻度と血管特性の差異を比較検討した。2断面経食道超音波断層法により、上行大動脈,大動脈弓部,下行大動脈を可及的に描出し,各々の部位の硬化病変の程度と定量的に分析した。また,門歯より約35cmの部位の下行大動脈の断層像を描出し,Mモ-ド法にて心周期における大動脈径変化を求めた。えられた血管Mモ-ド図より,血管硬化性の指標であるβを算出し,健常者群,FH群間で比較すると同時に,両者ともβと年齢との関係を求めた。〈結果〉FH群では,下行大動脈径には健常者群と差はみられないものの、血管硬化度を表わすβ値は有意に上昇(P<0.001)し,明らかな血管硬化が生じていることが判明した。また、硬化病変の定量評価においても、FH群では健常群に比し,有意に動脈硬化指数の増大がみられた。動脈硬化指数およびβ値と年齢の関係では、FH群においてもその関係式は明らかに急峻であり,健常者に比し,FH患者では年齢的に早期に硬化病変の発症がみられることが判明した。また,抗高脂血症薬の効果に関する研究は現在進行中であるが,数例においては約1年間の投薬後,明らかな動脈硬化病変の退縮が認められており,現在,症例数を増して検討中である。尚,この研究の一部は,第55回日本循環器学会総会ならびに日本動脈硬化学会に発表する予定である。
