An investigation on the application of a portable pulse oximetory to the home care

在家庭护理患者中，呼吸功能障碍患者的数量呈上升趋势。对安全的呼吸系统家庭护理程序的需求现在正在加强，特别是在患有神经系统疾病的患者中。对患者的呼吸和循环系统使用适当的监测设备是很有帮助的。本研究探讨便携式脉搏血氧仪在家庭呼吸照护中的适用性。本文对18例有呼吸功能障碍的神经系统疾病患者进行了研究，其中6例采用机械通气。结果如下：1. 为了进行安全的居家护理，呼吸监测是非常重要的，特别是当(1)呼吸功能障碍恶化时，(2)计划发展患者的日常运动，(3)做一些护理程序，如气管造口管的吸引。2. 当(1)长期观察呼吸功能时，(2)在护理期间防止SaO2减少时，(3)确定患者开展日常运动的时间表时，应用便携式脉搏血氧仪是有效的。3. 为了便携式脉搏血氧仪在家庭护理中的安全使用，应该需要(1)硬件/软件规格的改进（易于操作和安全误用），(2)用户培训系统，(3)家庭护理成员之间的信息网络，(4)开发其他适当的呼吸功能监测仪，可以与便携式脉搏血氧仪一起使用。

The number of patients with respiratory dysfunction has been increasing among the home care patients. The needs for the safety respiratory home care procedure is now intensifying, especially in the patients with neurological disease. It is very helpful to using some adequate monitoring devices for the respiratory and circulation system of the patient. In this research, we discussed on the applicability of the portable pulse oximetory to the home respiratory care. Eighteen patients with neurological disease were studied who had respiratory dysfunction and six of them were using mechanical ventilation. The results were as follows. 1. In order to carry out a safety home care, the respiratory monitoring was very important especially when (1) the respiratory dysfunction getting worse, (2)planning to develop patient's daily exercise, (3)doing some nursing procedure such as suctioning tracheostomy tube. 2. Application of the portable pulse oximetory was effective particularly when (1) long-term observation of respiratory function, (2) prevention of diminishing of SaO2 during under nursing, (3) determinate the schedule of developing patient's daily exercise. 3. For the safety use of portable pulse oximetory in home care, it should be needed (1) improvement of the hardware/software specification (easy to manipulate and safety to misuse), (2) training system for users, (3) information net work among the home care members, (4) development of other adequate monitors for respiratory functions, which could be using with the portable pulse oximetory.
