全結腸以上広範無神経節症ラットに対し、ileoendorectal pull through手術を行うとともに、腸管温存の部位別優劣を検討した。すなわち、無神経節腸管をパッチとして用いるが、部位により無パッチ群、右側結腸パッチ群、横行結腸パッチ群、左側結腸パッチ群、sham手術群の5群に分け、便性・便量、腸管通過時間、体重増加率、血液一般検査、血清総蛋白値、血清電解質およびCoopersteinの方法によりパッチ部位における水分・電解質吸収能を比較検討した。便性ではsham手術群以外は軟便で、便量では無パッチ群が他の群に比べ有意に多く、また会陰部に膿瘍形成などの病変を示した。腸管通過時間でも無パッチ群が他の群に比べ短かった。術後4週間の体重増加率では、無パッチ群ー30.9±3.7%、右側結腸パッチ群が+5.1±1.7%、横行結腸パッチ群で-3.4±4.0%、左側結腸パッチ群で-1.8±4.2%、sham手術群で+12.7±1.5%で無パッチ群に対し、他の群が有意に良好であった。血液検査では、ヘモグロビン値は各群に有意差はなく、またNa濃度は無パッチ群で139.8±1.8mEq/Lで右側結腸パッチ群に比べ有意に低値を示した。K濃度、C1濃度については各群間に有意差はなかった。血清総蛋白濃度ではやはり無パッチ群で低値を示し、右側結腸パッチ群、横行結腸パッチ群、sham手術群との間に有意差を示した。次にパッチ部位における水分・電解質吸収能の測定の結果、無パッチ群に比しパッチ群で吸収率は良好で、パッチ群の中でも右側結腸パッチ群の吸収率が良好であった。以上のように今回の検討から、右側結腸温存が他の部位の温在に比べ優れていることが示唆された。
