歯科領域での在宅医療制度が十分でないために歯科医療を受ける機会が少ない。いわゆる寝たきり老人対照として、口腔環境改善の指針を得る事を目的として老人専門病院と特別養護老人ホ-ムの2か所で口腔内の状態を補綴学的見地より調査した。調査項目は一般的調査として現在の疾患名、寝たきりの原因となった疾患名、日常生活動作(A.D.L)食事の状況、性格類型などである。歯科的調査として欠損様式、義歯の形態、残存歯数、齲蝕の程度、歯牙の動揺度、歯石沈着の有無、歯周治療必要度指数(CPITN)、歯肉の状態(G.I)、粘膜疾患の有無、顎堤粘膜の被圧縮度、歯科補綴物の有無、口腔及び義歯の清掃方法、総合的な咀嚼能力などを調査した。また、義歯装着可能な被験者3名を選択し、義歯装着前後の咀嚼能力についても調査した。結果の概要1.被験者は老人病院38名、老人ホ-ム23名で男性14名女性47名の計61名で平均年齢は76.2才である。2.全被験者に対する無歯顎者率は老人病院では63.2%、老人ホ-ムでは34.8%であった。3.有歯顎者29名の総残存歯数は269歯で、そのうち63歯はC_4で一人平均残存歯数は9.3歯であった。4.義歯が必要にもかかわらず持っていない者は25名で、そのうち治療を希望する者は僅か16%であった。5.食事の状況は普通食が41.0%、きざみ食52.5%、ミキサ-食4.1%、流動食1.6%であった。6.ストマスタットによるカンジダ菌検出結果は、陽性41.0%疑陽性18.0%,陰性41.0%であった。7.義歯が良好と判断された被験者の簡易咀嚼能力検査表でのポイントは平均15.1であった。8.義歯の装置により明らかに咀嚼能力は向上した。

Summary of results 1. 38 patients were hospitalized for the elderly, 23 for the elderly, 14 for men, 47 for women, and 61 for an average age of 76.2 years. 2. The total number of patients with no teeth was 63.2% in geriatric hospitals and 34.8% in geriatric hospitals. 3. The number of teeth remaining was 269 in 29 patients, and the number of teeth remaining was 9.3 in 63 patients. 4.
