在宅歯科診療の対象となる患者の全身状態や口腔内の状態は健常者と比較して不良となることが過去の研究結果より確認されている(文部省科学研究費補助金課題番号05671644)。そのため、通常の診療回数を減らすという歯科治療の簡略化が必要となってくる。そこで今回我々は、鋳造操作など、診療回数の増加する方法を極力避けて行う診療方法を試みた。少数歯残存の部分床義歯については、装着後のメインテナンスを考えて根面アタッチメントを用いる方法にし、残根上義歯を装着している患者に対しては、残根歯を積極的に維持歯として用いることとした。結果の概要:維持歯の破折やカリエス等により維持不良となり、義歯の機能が損なわれた症例を選択し、我々の開発した鋳造を必要としない維持棒付きの根面板(磁性合金)を使用し、義歯の即日改修を行った。術式としては、歯牙を歯肉縁にて切断し、合釘を形成し適当な長さに切断した維持棒付きの根面板をビスタイトセメントにて合着する。義歯側には磁性アタッチメントマグフィットEXを旧義歯内面に設置し咬合面部を形成しオーバーデンチャータイプとした。これにより、義歯の維持力は格段に向上し、摂取可能な食品数の増加が、当講座にて考案し、使用している簡易咀嚼能力検査表の評価でも確認できた。次にこの方法を、残根上義歯を装着していた、いわゆる寝たきり高齢者に適用した。残根歯のうち維持歯として利用できるものはかなり限られてくるが根面アタッチメントを適用した症例では維持力の増加が明らかに確認でき、咀嚼能力も向上した。
