《実験方法》本研究の趣旨を説明し同意の得られた、年齢18歳から24歳までの健康成人12名を対象とした。使用局所麻酔薬として、歯科臨床で汎用される(1)1/8万エピネフリン含有2%リドカイン(以下EL-1/8)、(2)1/20万エピネフリン含有2%リドカイン(以下EL-1/20)、(3)0.03%フェリプレッシン含有3%プロピトカイン(以下EP)の3種類を使用した。局所麻酔薬は上顎中切歯歯肉頬移行部より0.5mlを22G針を用い傍骨膜注射法で注入した。刺激方法は上顎中切歯に円盤電極を装着し0.1ms単発電気刺激を1回/1sec与えた。測定は脳の内的活動や心理過程に関連して変動するSEPの後期成分のうち約150msの陰性波をN1、約250msの陽性波をP2とし、N1-P2の振幅をA成分として測定し、痛み感覚の客観的評価とした。《結果》SEPのA成分は、全局所麻酔薬群で麻酔前に比べ局所麻酔注入2分後には減少し、5分後には3群でA成分の消失がみられた。EL-1/8およびEL-1/20では60分後でもA成分は消失していた。しかし、FPは60分後にはA成分が回復し始めた。各群の局所麻酔薬注入前のA成分の値を100としたA成分の経時的変化は、EL-1/8の各時間の値を対照とした場合、EL-1/20では局所麻酔注入後100分後まで有意な差はみられなかった。しかしFPでは注入40分後から85分後まで有意な差がみられた。(p<0.05)。このことから、EL-1/8はFPと比較し有意に麻酔効果及び効果時間が延長するが、EL-1/20との間には麻酔効果および効果時間に有意の差がみられないものと思われた。

The purpose of this study is to illustrate the results of the study in 12 healthy adults aged 18 and 24. The drug is widely used in local anesthesia, dentistry and clinical practice. Three types of drug are used: (1)1/80,000 drug containing 2% drug (EL-1/8 below),(2)1/200,000 drug containing 2% drug (EL-1/20 below), and (3)0.03% drug containing 3% drug (EP below). The local anesthetic was injected into the buccal transition part of the maxillary middle incisor with a 22-gauge needle. Stimulation method: Install the electrode of the maxillary middle incisor to generate electrical stimulation for 1 cycle/1sec. The activity and psychological process of SEP were measured. The negative wave of SEP was about 150ms, and the positive wave of SEP was about 250 ms. The amplitude of SEP was about 250ms. The A component of SEP was measured. The results showed that SEP A component decreased after injection of 2 minutes and disappeared after injection of 5 minutes. EL-1/8 EL-1/20 After 60 minutes, the A component is restored. When the value of component A before injection of the local anesthetic drug in each group is equal to 100 minutes, the time-based change of component A is compared with the value of EL-1/8 at each time. When the value of EL-1/20 is equal to 100 minutes after injection of the local anesthetic drug, the intentional difference becomes noticeable. After 40 minutes of injection, after 85 minutes, the difference is intentional. (p<0.05)。The EL-1/8 FP has been compared with the EL-1/20 FP. The EL-1/8 FP has been compared with the EL-1 FP.
