頭頚部腫瘍の病態へのepstein-Barr(EB)ウィルスの関与を検討する研究の一歩として、本年度は以下の検討を行なった。1.正常扁桃組織中でのEBウィルスゲノムの検出、ならびにアポトーシスの阻害と関連してEBウィルスのlatent membrane protein(LMP)-1の構造遺伝子の検討。(1)扁桃肥大39症例について扁桃組織中のEBウィルスの検出をpolymerase chain reaction(PCR)法を用いて行なった。プライマリ-はEBウィルス構造遺伝子の中からBamW領域より選定した。その結果、7割近くの症例で検出され、成人では22例中17例に(このうち、習慣性扁桃炎症例では83%に、単純肥大例では72%に)認められ、一方、小児17症例では59%に検出された。(2)EBウィルスのLMP-1がbcl-2の発現を亢進させることが知られているが、EBウィルスのアポトーシス障害による扁桃肥大の可能性を探るため、LMP-1構造遺伝子の検討をPCR-single strand conformation porphorism(SSCP)により検討した。しかし、慢性扁桃炎・単純肥大の扁桃組織から検出されるEBウィルスにより疾患特異的なパターンの存在は認められなかった。2.頭頚部腫瘍患者での唾液中のEBウィルスの検出術前照射治療を施行した頭頚部偏平上皮癌患者10症例より、唾液を採取しその中のEBウィルスゲノムの検出を行なった。術前照射により唾液中のEBウィルスの検出率は20%から60%に増加した。局所の免疫能の低下を反映するものと考えられ、その消長を検討することは、疾患の再発・予後との関連からも意義があるものと考えられた。さらに、今後は、頭頚部腫瘍組織中でのEBウィルスの検出、EBウィルスのLMP-1の機能発現の面からの検討を行ないたい。

A preliminary study of epstein-Barr(EB) disease in head tumors was conducted this year. 1.
