项目摘要

平成6年度の科学研究費に基づく研究概要は以下のとおりである。研究計画調書に沿って、1993年1月から1994年6月までの1年6ヶ月の間に顎関節疾患の臨床診断の下に放射線科に各種X線検査の依頼があった患者を検索の対象とした。放射線科に保存してあるアンケート及び診査記録を基にしてデータベースを作成し、コンピューター入力を行った。1年6ヶ月の間の新来患者数は614名であった。そのうち、顎関節断層撮影を行った患者数は、446名(72.6%)であった。その中で期間内に2回以上の断層撮影を行った患者数は33名(5.4%)で、そのうち10名については、MRI検査も行われていた。初回撮影日と最終撮影日の間隔は、平均で228日(約7ヶ月半)であった。初回撮影日に、下顎頭の形態変化が認められるとその後の撮影時には高率で、骨吸収性あるいは骨増生性の形態変化の進行が確認された。これは、断層撮影により観察した時期が形態変化の発生した時期と時間的な一致を見たものと考えられる。一方、初回撮影日に形態異常が認められても、その後に著しい変化を呈しない症例も存在した。下顎頭の骨形態変化は、関節円板の位置異常、形態異常あるいは動態異常とも密接に関係しており、下顎頭の形態変化の進行により代償的に臨床症状の漸善をみることがある症例が存在した。下顎頭の経時的な形態変化の進行のメカニズム等については未だ不明な点が山積しており、それらの点について、今後更に明らかにして行きたい。
