项目摘要

1.研究対象の変更点当初の研究計画では、対象を1991年にフッ化物洗口を開始した沖縄県久米島の具志川村、仲里村としていたが、比較検討するためには群間比較も有用と考えた。そこで対照地区として僻地的性格をもつ石垣島と都会的性格をもつ豊見城村(那覇市に隣接)を加えた。4地域の小学校1および6年生を対象に歯科検診および保護者対象に質問紙法調査を行った。2.研究結果の概要1)フッ化物洗口法の永久歯う蝕予防効果(1)DMF者率:1年生のDMF者率は具志川村30.9%、仲里村40.2%、石垣島36.3%、豊見城村44.8%であった。統計的には具志川村と豊見城村との間で有意差を認めた(p<0.05)。6年生では何れの地域でも90%を越えており、差は認められなかった。(2)DMFT-index:1年生のDMFT-indexは具志川村0.59、仲里村0.83、石垣島0.61、豊見城村1.02であった。6年生ではそれぞれ4.77、4.87、3.99、5.78であった。1年生では具志川村と豊見城村間に有意差を認めた。6年生では具志川村と仲里村間を除く全ての群間で有意差を認めた。群内比較では具志川村小学校1年生においては1991年の1.27から0.59と約半分に、6年生では5.77から4.77と約1歯減少しており、3年経過ながらフッ化物洗口法は、着実に効果を発揮していた。2)歯科保健に関する知識、意識、行動生活習慣に地域差は認められなかったが、具志川村6年生における歯痛経験者が対照地区の石垣島、豊見城村同年齢児に較べ、有意に少なかった。歯科保健に関する意識において、、具志川村・仲里村の保護者共に、「最近むし歯が減少している」と回答した割合が対照地区に較べて有意に高く、これはフッ素洗口に対する期待の現れ、効果の認識によるものと推察された。またこのことは、よ蝕予防の優先順位で具志川村・仲里村の保護者が「フッ素利用」を上位にあげていることによっても裏付けられた。
