As the liver has dual blood supplies, arterial and portal, regional hepatic blood flow had not been measured quantitatively. However, we succeeded in simultaneously measuring both regional hepatic arterial and portal blood flow by PET in non-stressed patients. To examine the more accurate physiological function of the liver, a new model was designed for positron emission tomographic study using H_2^<15>O,which was consisted of the components of the liver and the portal system, applied "curve analysis", and introduced a portosystemic shunt factor and a coefficient for circulation time within the portal organs. PET scans of the liver were performed using the H_2^<15>O dynamic method in 121 patients which were classified into 4 groups ; normal, chronic hepatitis, cirrhosis and the others. They were consisted of 61,39,16, and 5 patients. Significant differences between normal and cirrhotic patients were noted in the regional total, arterial, portal hepatic blood flow, the ratio of porto-systemic shunting, and the circulating time within the portal organs for the blood. There was a significant differences between them also in a coefficient of the liver for water when positron emission tomographic scan, which was thought to be useful as a new pathophysiological parameter of the liver. Positron emission tomographic study was beneficial for the physiological regional hepatic function test.
