平成6年度は、16症例に対してインターフェロン(IFN)とオフロキサシン(OFLX)の併用療法を施行した。このうちの11例で治療中のHCV-RNAタイタ-の推移を、マルチサイクリックRT-PCR方で経時的に測定し得た。11例のHCVジェノタイプの内訳は、HCV-11型が9例、IIIおよびIV型が各1例であった。HCV-II型のうちの3例がRNAの陰性化が見られない治療抵抗群で、1例がIFN単独では改善せずOFLXの追加投与によってRNAが陰性化した部分反応群であった。残りの3例とIIIおよびIV型の各1例は、OFLX開始前にRNAが陰性化した良好群であった。しかし、良好群中のHCV-IIの1例では、OFLX終了後のIFN治療中にRNAの再陽転を認めた。以上の結果より治療抵抗群と部分反応群にIFN抵抗性HCV株が存在すると推定されるが、いずれもHCV-II型の症例で、HCV-III・IV型にはIFN抵抗株は存在しないか、存在しても稀であることが考えられた。つぎに、治療抵抗群と部分反応群の4例でHCV-RNAのquasispecisの経時的変化について検討した。今回は、IFNの感受性を決定づけるとされているNS5A領域でsingle strand conformation polymorphism(SSCP)法を施行した。しかし、症例数が少ないためIFN抵抗株を決定づけるほどの共通点は見いだせず、抵抗群と部分反応群相違を明確にすることも不可能であった。そこで併用療法症例追加して検討している。また、OFLX終了後に再発した1例で、既存の報告と比べてHCV遺伝子配列に特徴がないか検討中である。

There was 1 case of HCV-II in good group and RNA in IFN treatment after OFLX was cured. The above results showed that in the treatment of partial anti-group IFN-resistant HCV strains, there were presumptive symptoms, cases of type II HCV-, type IV IFN resistant strains, and presence of antigens and antigens. One more example will be reissued after OFLX and the existing report will be compared to the existing HCV report with a list of special reports.
