急性上腸間膜動脈血栓塞栓症・非閉塞性腸管虚血の早期確定診断は、現在血管造影に頼らざるを得ないが、スクリーニングとしては侵襲的すぎる。最近の超音波断層装置にはドプラ機能が付帯されており、腹部アンギナなどの診断に有用であった症例報告は散見されるものの、血栓塞栓の位置や血管攣縮によるドプラ波形の変化に関する研究はみられない。そこで実験動物を使用し、上腸間膜動脈を近位部から順次閉塞、および血管収縮剤を使用した際のドプラ波形についての定量的解析が本研究の目的である。ドプラ機器は東芝製SSA-250A,3.75MHzコンベックスプローベである。まず、実験動物(体重20Kg前後の雑種成犬)6匹をペントバルビタール麻酔下に経腹壁的に上腸間膜動脈を検出した。次に上腸間膜動脈起始部から2cm、中結腸動脈、右結腸動脈、回結腸動脈分岐毎に、5Fバルーンカテーテルを用いた閉塞時と、エピネフリン持続動注による狭窄時のドプラ波形計測と血管造影を行なった。得られたドプラ波形について、最高流速、最低流速、平均流速、Acceleration lndex(Al),Palsatility Index(Pl)を算出した。まず回結腸動脈分岐部閉塞では、各数値ともに平均1%以下の変動にすぎなかった。右結腸動脈分岐部閉塞では、順に平均2.2、5.1、8.2、0.5、15.5%の減少が、中結腸動脈分岐部閉塞では同3.5、35.5、10.2、2.0、38.5%の減少が、上腸間膜動脈起始部から2cmの閉塞では同10.1、85.3、99.8、5.8、70.5%の減少が認められ、上腸間膜動脈の閉塞部が近位になるほど上腸間膜動脈根部でPlの減少が著明となることが示された。エピネフリン持続動注による狭窄実験は、注入部における攣縮が強すぎるため、アンギオテンシンなどより末梢血管を収縮させる薬剤での実験を予定中である。また今回の検討ではまだ対象が少なく、統計的検討を行なうためにさらに実験を重ねる予定である。

The origin of the secondary mesenteric artery is 2 cm, the middle colon artery, the right colon artery, and the ileocolon artery are branched, 5F, and 5 F. Calculate the maximum velocity, minimum velocity, average velocity, Acceleration lndex(Al), Palatability Index(Pl). The average number of patients in the iliac artery is less than 1%. The average decrease of Qian Hong in the bifurcation of right colonic artery was 2.2, 5.1, 8.2, 0.5 and 15.5%. The decrease of Qian Hong in the bifurcation of middle colonic artery was 3.5, 35.5, 10.2, 2.0 and 38.5%. The decrease of Qian Hong in the origin of superior mesenteric artery was 2cm and 10.1, 85.3, 99.8, 5.8 and 70.5%. The proximal part of the superior mesenteric artery is located in the middle of the upper mesenteric artery. The root of the superior mesenteric artery is located in the middle of the upper mesenteric artery. For example, if a patient has a heart attack, he or she may be able to recover from it. This year's survey is scheduled to be conducted on a statistical basis.
