本年度は、103名(男性76、女性27)の腎移植患者について、移植後のせん妄の有無と、年齢、移植前の透析期間、移植腎、免疫抑制剤との関係について解析を行った。[結果]全体で16.5%(17名)の腎移植患者にせん妄を認めた。軽度の意識混濁と幻覚を認めた患者は4名、ベッドから起きあがる等の行動を示したが精神科的対応の必要なかった患者は6名、興奮や不穏が激しく、身体抑制や向精神薬で対応した患者は7名であった。せん妄群(n=17)の平均年齢は39.1歳、非せん妄群(n=86)では30.7歳で有意差(p=0.0012)を認めた。せん妄群の平均透析年数は4.4年、非せん妄群では2.0年で有意差(p=0.0033)を認めた。死体腎移植(n=31)では38.7%(n=12)の患者にせん妄が出現し、生体腎移植(n=72)では6.9%(n=5)で、有意差(p=0.0002)を認めた。免疫抑制剤にazatioplineを使用した患者(n=41)では2.4%(n=1)、cyclospolineAやFK506を使用した患者(n=45)では35.5%(n=16)にせん妄が出現し、有意差(p=0.01)を認めた。性差やHDとCAPDでの比較は有意差はなかった。cyclospolineA使用例(n=59)に限定した解析では、非せん妄群の平均年齢は31.9歳、せん妄群では39.8歳で有意差(p=0.01)を認めた。透析年数の比較では有意差を認めていない。せん妄の発現率は、死体腎(n=25)では40%(n=10)、生体腎(n=34)では14.7%(n=5)であり、有意差(p=0.0367)を認めた。[考察]腎移植後のせん妄の発現には、移植時の年齢、移植腎の種類、免疫抑制剤の種類、移植前透析年数などが関与している。今後はさらに、免疫抑制剤の量、職業や家族背景などを検討項目に加え、重回帰分析による解析を試み、移植後の精神状態やQOLの予測に役立つ評価スケールの作成につなげたいと考えている。
