対象と方法神奈川県西部に位置する4つの単科精神病院の医療スタッフを対象にチーム医療評価尺度への回答を依頼した。課題遂行のための情報(医学情報、情緒、ニーズ)の処理、集団維持のための情報(スタッフ情報、情緒、ニーズ)の処理、上司からの支持、医師からの支持、コミュニケーション、教育、身体的疲労感、精神的疲労感、退行的集団心理といった下位尺度とチーム医療との相関関係を調査した。次に13の下位尺度を独立変数、チーム医療度を従属変数として重回帰分析を施行した。結果A病院は125名、B病院は69名、C病院は97名、D病院は40名の合計331名(医師31名、看護スタッフ239名、その他の医療スタッフ61名)の医療スタッフから回答を得た。チーム医療度は、患者や家族の情報の処理(相関係数=0.68)、ニーズの処理(0.67)、情緒の処理(0.66)、集団の情報の処理(0.68)、集団のニーズの処理(0.69)、集団の情緒の処理(0.61)と高い正の相関を示した。また教育と高い相関(0.71)を示した。医師の支持との相関(0.61)が上司の支持(0.46)との相関よりも高かった。身体的疲労(-0.37)、精神的疲労(-0.33)、退行的集団心理(-0.57)とは負の相関を示した。重回帰分析(増加ステップワイッズ)の結果、寄与率62%の回帰式が得られた。変数と回帰係数は教育(0.22)、患者と家族の要求の処理(0.18)、医師の支持(0.16)、コミュニケーション(0.1)、集団に関する情報の処理(0.07)、身体的疲労(-0.046)であった。考察チーム医療が円滑に営まれているいくためには、医療行為や看護行為などの課題遂行のための情報処理に加え、集団維持するための情報処理が円滑に機能していることが重要である。それに加え、医師からの医療スタッフへの支持と教育的な関与が重要になる。また疲労感や退行的集団力動を軽減させるような配慮が必要になることが理解できた。
