[対象と方法]本年度は、1990.9〜1995.8までの5年間に東海大学大磯病院理学療法科を受診した神奈川県二宮町在住の65歳以上の患者124名の家族を対象に、郵送によるアンケート調査を行った。主介護者、その他の家族メンバーに記入してもらった。調査内容は家族環境尺度(FES)、自己評価式QOL質問表(QUIK)、日常生活能力などである。コントロール群として老人クラブに所属する65歳以上の老人120名の家族にも同様の調査を行った。[結果]障害者家族に関するFESの有効回答は53(発送数全体の44%)であり、障害者の既往疾患は脳血管障害20、整形疾患33であった。QUIkの有効回答は58(48%)であり、脳血管障害20例、整形疾患35、その他3であった。健康老人群における有効回答は87(72.5%)であった。障害者家族のFES各項目の平均得点は、凝集性6.49±4.56、表出性6.00±3.84、葛藤性1.94±2.90、独立性4.71±3.36、達成志向性2.92±2.87、知的文化的志向性4.84±5.24、活動娯楽志向性3.88±4.17、道徳宗教性4.71±3.51、組織性6.39±4.01、統制性3.66±2.69であった。この結果について、健康老人家族の平均値と比較しt検定した結果では、活動娯楽性が有意に低くなっていた(p<0.005)。既知である世田谷区の思春期家族の平均値と比較して信頼区間95%でt検定した結果では、葛藤性が有意に低く、達成志向性、道徳宗教性、組織性は有意に高かった。主介護者のQUIKの平均得点は合計6.8±8.4、身体機能2.8±3.1、情緒適応1.4±2.0、対人関係1.1±1.9、生活目標1.3±2.1であった。健康老人の家族メンバーとの比較では統計学的な有意差は認めなかった。[考察]65歳以上の身体障害老人を持つ家族では、健康老人がいる家族より活動娯楽志向性が低かった。介護という状況が、家族の趣味や娯楽に費やす時間を制限したり、身体的心理的ストレスにより趣味娯楽そのものへの関心の低下が生じている可能性がある。QUIKの平均得点の比較では、健康老人家族との間に有意差はなかった。これは、今回の対象に障害が軽度の患者が含まれていたため、家族メンバーのQOLに差が生じなかったためと考えられる。今後は対象数を増やし、身体障害の重傷度と家族環境や介護者のQOLとの関係について検討する予定である。

[Methods] During the five-year period from September 1990 to August 1995, the Department of Physiology, Tokai University Oiso Hospital conducted a family survey of 124 patients aged 65 and older living in Niomiya Town, Kanagawa Prefecture. The main mediator, the other family members, are recorded in the record. The survey included Family Environmental Scale (FES), Self Assessment QOL Questionnaire (QUIK), and Daily Living Ability. A survey was conducted on the family identity of 120 elderly people aged 65 and older. [Results] There were 53 cases of FES related to the families of the victims (44% of the total number of transmission), 20 cases of vascular disorders and 33 cases of plastic disorders. QUIk has 58(48%) 87(72.5%) of healthy elderly people answered. The average score of FES items in families with disabilities was 6.49±4.56, 6.00±3.84, 1.94±2.90, 4.71±3.36, 2.92±2.87, 4.84±5.24, 3.88±4.17, 4.71±3.51, 6.39±4.01, and 4.71±3.51. Control 3.66±2.69. The results showed that the average value of healthy elderly was significantly lower than that of healthy elderly (p<0.005).
