慢性気道疾患患者における痰の喀出状態と関連する因子を解明するために研究を行い以下の結果を得た。1.喀痰の膿性度と基礎疾患の関係をみると、慢性気管支炎および気管支喘息では、主に粘液性痰を喀出している患者が80-90%であるのに対し、気管支拡張症とびまん性汎細気管支炎では70-80%の患者が主に膿性痰を喀出していた。2.痰の喀出状態に関する自覚症状について面接により調査した。1)痰の全体的な咯出状態は、粘液性痰喀出患者の半数が喀出困難を自覚し、その程度には個人差が認められた。膿性痰喀出患者の85%が咯出困難は軽いと回答した。2)咯出される痰の形状は、膿性痰群では85%が塊状と回答し、このうち90%が喀出容易と回答した。粘液性痰群では、形状は塊状は26%と少なく、糸をひく、水のようにさらさらしているなどの非塊状と回答した患者が74%みられ、塊状痰患者で喀出が容易、糸をひく痰喀出患者で喀出困難の割合が高い傾向がみられた。ただし、痰の形状と喀出困難との関係は膿性痰群に比較し粘液性痰群では複雑であった。3)痰喀出時のせきの程度が高度な患者の割合は粘液性痰喀出群で膿性痰喀出群より高くなっていた。4)痰が気道を閉塞する、あるいは胸がつかえる感じは、両群間の差は少なかった。3.痰の糸ひき性は膿性痰に比較し粘液性痰で有意に高値であった。粘液性痰喀出患者において、糸ひき性と痰の喀出状態と関連する自覚症状との相関を検討した結果、両者間に一定の関係が存在することが示唆された。以上の成績は、膿性痰群では痰の喀出自体は比較的容易であると自覚している患者が多いのに対し、粘液性痰群では痰の喀出困難な患者と軽度な患者があり、しかも患者間の差が大きいことを示している。従って個々の患者の痰の喀出状態を正確に観察し、それに対応した個別的な去痰療法を行う必要性は、膿性痰群より粘液性痰群でより高いと考えられる。
