In this project, changes of microbial flora from the subgingival pockets in the active phase of periodontitis were investigated.To clarify the characteristics of microbial flora in the acute phase of periodontitis which seems to be one of the active phase of periodontitis, microbial flora of subgingival pockets of scute phase was compared with that of chronic phase. The proportions of Porphyromonas gingivalis and Bacteroides forsythus against total CFU from acute sites were significantly higher than those from chronic sites. Those bacteria seemed to play important role in the acute phase of periodontitis. And the monitoring of these bacteria seemed to be important for the diagnosis of active phase.For the monitoring of those periodontopathic bacteria, more simple and swifter bacterial tests are required. For detecting periodontopathic bacteria, non-radioactive DNA probe method (Affirm DP<@D1(]SY.encircled.R[)@>D1) was evaluated. DNA probe method showed superiority in sensitivity and specificity for the detection of P.gingivalis and B.forsythus compared with cultural method. DNA probe method could be apply for the microbial diagnosis for active phase.In view of the MIC of minocycline-HCl for periodontopatic bacteria, P.gingivalis and Actinobacillus actinomycetemcomitans did not showed the low susceptibility to minocycline-HCl. For the effect of local application minocycline gel, clinical improvement and significant decrease of P.gingivalis and B.forsythus was obsereved by one application. One application seemed to enough for alleviation of acute phase. As some cases showed the recolonization of periodontopathic bacteria in 4 weeks, debridement of periodontal pockets seemed to be effective after clinical improvement was observed by topical application of minocycline.
本研究通过对活动期牙周炎患者龈下袋微生物区系变化的研究,比较了活动期与慢性期牙周炎急性期与慢性期的微生物区系,以明确牙周炎急性期与慢性期的菌群特征。急性部位牙龈卟啉单胞菌和福赛氏杆菌占总CFU的比例明显高于慢性部位。这些细菌似乎在牙周炎急性期起着重要作用。而对这些细菌的监测对于活跃期的诊断似乎很重要。为了监测这些牙周病细菌,需要更简单、更快速的细菌检测。对非放射性DNA探针法检测牙周病原菌进行了评价。与培养法相比,DNA探针法在检测牙龈假单胞菌和连翘假单胞菌方面具有更高的敏感性和特异性。DNA探针法可用于活跃期的微生物诊断。鉴于牙周病细菌对盐酸米诺环素的MIC,牙龈假单胞菌和伴生放线杆菌对盐酸米诺环素的敏感性并不低。局部应用米诺环素凝胶,一次用药可明显改善临床症状,减少牙周炎和连翘杆菌感染。一次敷贴似乎就足以缓解急性期。局部应用米诺环素后,部分病例在4周内出现牙周病原菌再定植,牙周袋清创效果明显。
