今年度は空間恐怖の有無によるPanic Disorder(PD)患者の生物学的特徴を、聴覚事象関連電位((1)認知機能の指標:P300成分、(2)中枢Serotonin神経系機能の指標:the slope of the amplitude/stimulus-intensity function(ASF slope))を用いて治療前と認知療法導入後で検討した。対象と方法:DSM-IVの診断基準を満たす未服薬のPD患者のうち、検査について十分な説明を行い同意の得られた39例で、空間恐怖を伴う(AP+)群19例、伴わない(AP-)群20例。うち認知療法終了再測定しえた症例はAP+群5例,AP-群6例。対照群は健康成人18例。P300測定は日本脳波筋電図学会の測定指針に基づき、75dB,2KHz(標的)及び1KHz(非標的)tone burstを2:8の頻度で呈示し、標的刺激を数えるoddball課題とし、Pz導出波形を30回加算平均した。ASF slopeは,1KHzで音圧の異なる4種類のclick音(55,65,75,85dB)を各40回呈示し、音圧ごとの加算平均波形のN1/P2電位差を求め、最小自乗法によりその傾斜を算出(単位:μV/10dB)した。結果:(1)治療前の測度。P300潜時はAP+群でAP-群(p<.05)と対照群(p<.01)よりも有意に延長していたが、AP-群と対照群の間には有意差を認めなかった。P300電位は3群間に有意差を認めなかった。ASF slopeはAP+群で他の2群より急峻な傾向を認めたが統計学的有意差はなかった。(2)認知療法導入後の測度。例数は少ないが、各群間にみられた傾向は治療前と同様で、認知療法による有意の変化は認められなかった。考察:昨年度の結果からAP+群の生物学的異質性の可能性を指摘してきたが、今年度の認知療法治療前後を通じたAP+群でのP300潜時の延長所見からAP+群が他の群とは異なった認知機能の特徴を素因として有していることが推測された。また、AP+群の急峻なASF slopeは中枢Serotonin機能が他の2群よりも低下している可能性が推測された。これらはAP+群の生物学的異質性の可能性を示唆する所見と考えられる。研究結果の一部を第8回日本総合病院精神医学会総会、第11回国際事象関連電位会議などで報告した。

The biological characteristics and event-related potentials (1) cognitive function index:P300 component,(2) central Serotonin neurological function index:the slope of the amplitude/stimulus-intensity function(ASF slope)) of Panic Disorder(PD) patients were investigated before and after cognitive therapy. Methods: The diagnostic criteria of DSM-IV were examined in 39 PD patients, AP+ group in 19 patients, AP-group in 20 patients. After cognitive therapy, 5 cases were AP+ group and 6 cases were AP-group. 18 healthy adults were examined. P300 measurement is based on the measurement index of Japanese Institute of Electrical and Mechanical Engineering, 75dB,2KHz(standard) and 1KHz (non-standard)tone burst = 2:8, frequency of presentation, number of oddball subjects, Pz derived waveform = 30 times plus average. ASF slope: 1KHz sound voltage difference: 4 types of click sound (55,65,75,85dB): 40 times each; sound voltage difference: N1/P2 potential difference of the added average waveform; minimum self-calibration method:μV/10dB): Results:(1) Measurement before treatment. The number of cases is small, the tendency of each group is small, the tendency of cognitive therapy is small, and the tendency of cognitive therapy is small. To investigate the possibility of biological heterogeneity of AP+ group in yesterday's results and the prolongation of P300 latency in AP + group before and after cognitive therapy in this year's results. The AP+ group has a high level of ASF slope and the central Serotonin function has a low level of AP + group. This study shows the possibility of biological heterogeneity in AP+ populations. The results of this study were presented at the 8th Japan Psychiatric Association Conference and the 11th International Conference on Event Related Potential.
