パニックディスオーダー(PD)患者群を対象に、聴覚事象関連電位のP300成分と、空間恐怖・治療反応性について検討した。対象患者群はDSMIII-Rの診断基準を満たし、目的・方法を説明し同意を得た35名(男13名、女22名、平均35.9±7.3才)で、対照群は健康成人17名(男13名、女4名、平均32.4±2.1才)とした。1)空間恐怖の有無による認知機能の電気生理学的特徴の検討。患者群の亜型分類は、空間恐怖を伴う群(A+群)18名、伴わない群(A-群)17名。検査は服薬前又は1週間の未服薬期間をおいて実施し、病像の評価は問診とSDS,STAI,HRSD,HRSAにより施行した。事象関連電位は音刺激によるodd-ball課題とし、目標刺激の計数課題を遂行させた。頭皮上Fz、Cz、Pzの3部位から導出した波形をNeuropac k8(日本光電製)を用いて、帯域周波数1〜50Hzで30回加算平均した。P300の潜時と電位は、A+群、A-群、対照群の順に、Fzでは350.8±19.4msec,6.8μV、333.6±16.1msec,7.3μV、325.8±14.6msec,7.6μV、Czでは356.2±24.9msec,8.8μV、335.8±20.3msec,8.1μV、331.9±23.0msec,10.7μV、Pzでは364.8±19.2msec,11.1μV、345.1±20.6msec,10.2μV、337.9±23.0msec,12.8μVであった。潜時についてA+群は、A-群及び対照群との間にFz,Cz,Pzの3部位で有意差を認めた。患者群の病像の評価点は2群間で差がなかった。2)P300潜時の治療による変化。アルプラゾラムを4週間投与し、PD症状の改善した患者群(A+群12名、AP-群10名)において、P300成分潜時、電位の4週間の変化を測定したが、P300潜時・電位の変化分については両群で有意差はなかった。1)2)より、A+群は電気生理学的に把握可能な認知機能障害を有した群であり、PD患者の空間恐怖の素因規定性の可能性が示唆された。今後は認知療法を適用した患者群についても治療反応性と生理学的測度の変化を検討し、電気生理学的特徴からの亜型分類を試みる。
